      Effect of Thermocycling on Color, Surface Characteristics, Compressive Fracture, and Fracture Modes of Prosthetic CAD/CAM Ceramics After Smokeless Tobacco Immersion

          Abstract

          Background

          The study aimed to assess effect of smokeless tobacco (ST) immersion with thermocycling (TC) on color of Vita Classical shade-guide, surface roughness (Ra) and roughness depth (Rz), and compressive fracture force (CFF) of hybrid polymer-infiltrated-feldspathic ceramic (Vita Enamic), leucite-Feldspathic glass (Vitablocs ® Mark II), and zirconia-reinforced lithium silicate (Vita Suprinity) CAD/CAM prosthetic ceramic materials.

          Material/Methods

          A 48 samples were milled from Vita Enamic, Vita mark II, and Vita Suprinity. VitaPan shade-guide, Ra, and Rz were registered before ST immersion. Then, the same parameters were recorded again after 15 days of ST staining with and without TC. CFFs and fracture modes of samples after 3 months of aging were documented.

          Results

          A slight change was observed in VitaPan shade-guide. Ra and Rz of Vita Suprinity (VS) recorded the highest values among ceramic groups at 3 time intervals. Increases in Ra and Rz were observed after immersion and staining with TC. ANOVA and Bonferroni post hoc tests were used. Significant differences among and between groups were noticed only before immersion in ST and TC, with P value ≥0.00. The highest mean value and SD of CFFs (MPa and Newton) were recorded in Vita Suprinity, followed by Vita Enamic (VE) and Vita mark II (VMII). Reparable fracture was higher in VMII and VS.

          Conclusions

          ST staining and TC caused color changes in VitaPan shade-guide and increases in Ra and Rz of tested materials, with highest and lowest values recorded in VS and VE. CFFs were marginally equal to clinically accepted values for VS and equal or slightly higher than the half values for VE and VMII groups.

          Assessment of perceptibility and acceptability of color difference of denture teeth.

          To assess the color difference values that represent the perceptibility and acceptability thresholds of denture teeth. Eighteen interchangeable right upper central incisors denture teeth were selected to have color difference values from representative left upper central incisor denture tooth in a range of 0.4-11.4 ΔE* units measured with a spectrophotometer (CM 2600d, Minolta Konica, Japan). These teeth were placed in an artificial dental arch in a phantom head. Under controlled conditions, a total of 80 observers divided into 4 groups (technicians, nurses, dentists and researchers) were asked to report the perceptibility and acceptability of each of the color difference values. The findings was then analysed using logistic regression for determining the perceptible and acceptable thresholds of dental color differences. The determined color difference values at which 50% of all observers could detect a color difference (50:50 probability) was 1.9 ΔE* units with a 95% confidence interval and ranged from 1.7 ΔE* units to 2.1 ΔE* units. The determined color difference value at which 50% of all observes preferred to replace the tooth because of unacceptable color difference (50:50 probability) was 4.2 with a 95% confidence interval that ranged from 3.9 ΔE* units to 4.7 ΔE* units. Within the limitation of this study, the following conclusions were made: 1. The mean color perceptibility threshold was significantly lower than the mean color acceptability threshold. 2. Significant differences between different observers groups were noticed in determination of perceptibility and acceptability of color differences. The technicians groups displayed lower perceptibility and acceptability limits than the dental nurses and non-dental observers. When shade matching artificial denture teeth to natural teeth, 50:50 perceptibility (1.9 ΔE* units) and acceptability (4.2 ΔE* units) thresholds are used to compare color difference values.             Smokeless tobacco (shammah) in Saudi Arabia: a review of its pattern of use, prevalence, and potential role in oral cancer.

            Shammah is a traditional form of chewing tobacco [smokeless tobacco, (ST)] that is commonly used in the Middle East especially in Saudi Arabia (KSA), Yemen and Sudan. The Substance Abuse Research Centre (SARC) at Jazan University noted that no adequate research and information on the prevalence of shammah use in the province of Jazan, and KSA as well, has been provided in the scientific literature.

              Effect of the Composition of CAD/CAM Composite Blocks on Mechanical Properties

              The aim of this study was to evaluate the effect of the composition of CAD/CAM blocks on their mechanical properties. Nine different CAD/CAM blocks, enamel and dentine, were tested. Sixteen samples of each material were separated for Vickers microhardness test (n=6, 5 readings per specimen), nanohardness test (n=6, 5 readings per specimen), filler weight (n=3), and SEM imaging (n=1). Data were statistically analysed using one-way ANOVA. Vita Mark II ceramic showed significantly higher values of hardness (in both nano- and microscale) and elastic modulus (6.83 GPa, 502 kg/mm2, and 47.7 GPa), respectively, than other materials. CAD/CAM composite blocks showed comparable values of hardness and elastic modulus to those of dentine but lower than those of enamel and ceramics. SEM images highlighted different filler-matrix microstructure of CAD/CAM composite blocks. It was concluded that (1) hardness and elastic moduli are positively correlated with ceramic filler percentage and microstructure and (2) CAD/CAM composite materials have comparable hardness and elastic moduli to tooth structure.
