          Abstract

          Introducción. El mieloma múltiple (MM) es una proliferación clonal de células plasmáticas caracterizada por la producción anormal de inmunoglobulinas o fragmentos de inmunoglobulinas y daño a órganos blanco. Afecta principalmente a pacientes ancianos, de sexo masculino y raza negra. Objetivo. Describir las características epidemiológicas, clínicas y laboratoriales de pacientes con MM en nuestro medio. Material y métodos. Estudio descriptivo, transversal basado en la revisión de expedientes clínicos de casos nuevos de MM diagnosticados en el servicio de Hematología del Hospital Materno Infantil de la Caja Nacional de Salud en la ciudad de La Paz durante un año (marzo 2012 a febrero 2013). Resultados. Se estudiaron 14 pacientes nuevos con diagnóstico de MM durante el periodo de estudio. La edad promedio fue de 57.7 años, con una mediana de 58.5 años y un rango de 38 a 74 años. La relación por sexo fue de 1:1. La mayoría fue procedente de La Paz. El tiempo promedio desde el inicio de las manifestaciones clínicas hasta el diagnóstico fue de 4.1 meses con un rango de 1 a 12 meses. La astenia, pérdida de peso, dolor óseo y lumbalgia fueron las manifestaciones más frecuentes. El 100% de los pacientes presentó anemia al momento del diagnóstico. La mayoría de los pacientes presentó elevación de la velocidad de eritrosedimentación y de las globulinas. El 64% de los pacientes presentó hipoalbuminemia. La variedad de mieloma más frecuente fue IgG seguida de IgA. El tratamiento más utilizado fue el esquema talidomida-dexametasona. Conclusiones. Se evidencian diferencias epidemiológicas, clínicas y laboratoriales en nuestra población comparadas con las reportadas en la literatura. Se requieren más estudios en el medio para corroborar estos hallazgos.

          Translated abstract

          Introduction. Multiple myeloma (MM) is a clonal proliferation of plasma cells characterized by abnormal production of immunoglobulinas or immunoglobulin fragments and target organ damage. It mainly affects elderly, male and black patients. Objective. To describe epidemiologic, clinical and laboratory features of patients with MM in our population. Material and methods. Descriptive, cross-sectional study based on a review of medical records of new cases of MM diagnosed in the Hematology service of the Materno Infantil Hospital (Caja Nacional de Salud) in La Paz city over one year (March 2012 to February 2013). Results. Records of 14 new patients diagnosed with MM were reviewed during the study period. The average age was 57.7 years, with a median of 58.5 years and a range of 38-74 years. The sex ratio was 1: 1. Most patients were from La Paz. The average time from the onset of clinical symptoms to diagnosis was 4.1 months with a range of 1-12 months. Fatigue, weight loss, bone pain and back pain were the most frequent manifestations. All patients had anemia at diagnosis. Most of the patients had elevated erythrocyte sedimentation rate and globulins. 64% of patients had hypoalbuminemia. The most common variety of myeloma was IgG followed by IgA. The most used treatment was the thalidomide-dexamethasone scheme. Conclusions. There are epidemiologic, clinical and laboratory differences in our population compared to those reportedin the literature. Further studies are required to corroborate these findings.

          International Myeloma Working Group consensus statement for the management, treatment, and supportive care of patients with myeloma not eligible for standard autologous stem-cell transplantation.

          To provide an update on recent advances in the management of patients with multiple myeloma who are not eligible for autologous stem-cell transplantation. A comprehensive review of the literature on diagnostic criteria is provided, and treatment options and management of adverse events are summarized. Patients with symptomatic disease and organ damage (ie, hypercalcemia, renal failure, anemia, or bone lesions) require immediate treatment. The International Staging System and chromosomal abnormalities identify high- and standard-risk patients. Proteasome inhibitors, immunomodulatory drugs, corticosteroids, and alkylating agents are the most active agents. The presence of concomitant diseases, frailty, or disability should be assessed and, if present, treated with reduced-dose approaches. Bone disease, renal damage, hematologic toxicities, infections, thromboembolism, and peripheral neuropathy are the most frequent disabling events requiring prompt and active supportive care. These recommendations will help clinicians ensure the most appropriate care for patients with myeloma in everyday clinical practice.
