      Trajectory of the Victims of Overland Transport Accidents: From Prehospital to Hospital Care

          Abstract

          Objective

          The aim is to evaluate the trajectory of the victims of ground transportation accidents from the prehospital care to the hospital.

          Methods

          The sample consisted of 1,264 patients collected from the prehospital care, from June 2015 to June 2016. The trajectory was analyzed by the difference in the average time elapsed between the call and the hospital entrance. The Mann–Whitney and Kruskal–Wallis tests were used, adopting a significance level of 5% and 95% confidence.

          Results

          Patients had a mean age of 31.2 years, Glasgow Coma Scale of 14.8 points, and 24.8 days of hospitalization. It is characterized predominantly masculine, conductive of the vehicle, being the motorcycle prevalent; most of them wore a helmet/seat belt and no alcohol. The association between the average time of care and the characteristics related to the victim was significant: the use of the belt/helmet, alcoholism, and type of discharge and relative to the accident: area of occurrence, period of the week, shift of occurrence, type of prehospital care, and other party involved.

          Conclusion

          The characteristics related to ground transportation accident interfere in the time of prehospital care to the hospital, which can influence the prognosis.

          Características de motociclistas envolvidos em acidentes de transporte atendidos em serviços públicos de urgência e emergência

          Resumo As lesões decorrentes dos acidentes com motociclistas constituem importante problema de saúde pública no Brasil. Objetivou-se descrever as características de motociclistas envolvidos em acidentes de transporte, atendidos em serviços públicos de urgência e emergência nas capitais de estado e no Distrito Federal. Estudo transversal com base nos dados do Inquérito sobre Violências e Acidentes em Serviços de Urgência e Emergência (VIVA Inquérito) de 2014. Os dados foram analisados segundo características sociodemográficas, do evento e do atendimento. Diferenças proporcionais entre os sexos foram analisadas pelo teste do qui-quadrado (Rao-Scott) com nível de significância de 5%. Do total de atendimentos de motociclistas, predominaram homens (n = 9.673), (razão de sexo = 3,2), jovens de 20 a 39 anos (65,7%), pretos/pardos (73,6%), com atividade remunerada (76,4%). Uso de capacete foi relatado por 79,1% das vítimas, 13,3% haviam consumido álcool nas seis horas anteriores ao acidente, 41,4% dos eventos estavam relacionados ao trabalho da vítima. Os acidentes foram mais frequentes nos finais de semana, durante a manhã e final da tarde. Estas características pode apoiar o desenvolvimento de políticas públicas de prevenção de acidentes e na promoção da saúde.

            Comparison of traffic-injury related hospitalisation between bicyclists and motorcyclists in Taiwan

            Objectives Bicyclists and motorcyclists contribute substantially to the morbidity and mortality rates of road crash casualties. The objective of the study was to investigate the crash characteristics of bicyclist and motorcyclist casualties presented to hospitals in Taiwan resulting from crashes. Methods By using linked data from The National Traffic Crash Dataset and the National Health Insurance Database between 2003 and 2012, logistic regression models were used to examine the determinants of hospitalisation among motorcyclist and bicyclist casualties. The examined variables include demographic characteristics, road and weather conditions, and vehicle characteristics. Results A total of 1,998,606 two-wheelers were enrolled in the study, of whom 216,600 were hospitalised: 203,623 were motorcyclists and 12,964 were bicyclists. Bicyclists were more likely to be hospitalised than motorcyclists were (14.0% vs. 10.7%). The pooled logistic regression model shows that bicyclists had higher odds of hospitalisation than motorcyclists (adjusted odds ratio [AOR] = 1.11, 95% confident interval [CI] = 1.08–1.14). In the motorcyclist and bicyclist models, helmet non-use appears to be a determinant of hospitalisation for motorcyclists (AOR = 1.14, CI = 1.12–1.16), although insignificant for cyclists (AOR = 1.03, CI = 0.94–1.12). Other important determinants of hospitalisation for motorcyclists and cyclists include female riders, elderly riders, rural roadways, unlicensed riding (for motorcyclists only), curved roadways, defective roadways, alcohol consumption (only for motorcyclists), and single-vehicle crashes (for motorcyclists only). Conclusions The result that bicyclists had an increased probability of being hospitalised than motorcyclists is particularly noteworthy, because there have recently been much more users of bikesharing systems in metropolitan cities where cycle helmets are not provided. We further found that helmet non-use was also a risk factor for motorcyclists, but insignificant for cyclists, possibly due to lower helmet utilization rates among bicyclists. Our findings regarding the increased hospitalisation percentage emphasize the importance of helmet use.

              Internações hospitalares por lesões decorrentes de acidente de transporte terrestre no Brasil, 2013: permanência e gastos

              Resumo Objetivo: descrever as taxas de internação por acidente de transporte terrestre (ATT) no Brasil em 2013, estimar gastos hospitalares e tempo de permanência das internações. Métodos: estudo ecológico com dados do Sistema de Informações Hospitalares do Sistema Único de Saúde (SIH/SUS). Resultados: em 2013, houve 170.805 internações por ATT financiadas pelo Sistema Único de Saúde, sendo 78,2% de indivíduos do sexo masculino, 48,6% na faixa etária de 20 a 39 anos e 51,9% motociclistas; a taxa de internação por ATT foi de 85,0 por 100 mil habitantes; o gasto total dessas internações alcançou R$231.469.333,13, com 1.072.557 dias de permanência e média de 6,3 dias de internação por paciente. Conclusão: a magnitude das internações decorrentes de ATT foi elevada; as vítimas internadas foram predominantemente homens, jovens e motociclistas; o tempo médio das internações foi de quase uma semana, implicando gastos expressivos.
