          Abstract

          ABSTRACT Objective To analyze the management actions of nurses in Primary Health Care services from the perspective of technologies. Method Descriptive-exploratory study with a quantitative approach, carried out from February to April 2019. Through the snowball technique, the sample consisted of 42 nurses from Primary Health Care, using an electronic tool for data collection. The data were analyzed using descriptive statistics techniques. Results 54.8% of nurses use technologies through SISREG and PEC electronic systems, care training related to permanent health education, online and face-to-face courses, in addition to care technologies such as telehealth and telemedicine. Conclusion Management actions focused on technology showed restricted use. Nurses need to empower themselves with these organizational devices, with the aim of improving the provision and quality of care for users.

          RESUMEN Objetivo Analizar las acciones de gestión del enfermero en los servicios de Atención Primaria de Salud desde la perspectiva de las tecnologías. Método Estudio descriptivo-exploratorio con abordaje cuantitativo, realizado de febrero a abril de 2019. Mediante la técnica de bola de nieve, la muestra estuvo conformada por 42 enfermeros de Atención Primaria de Salud, utilizando una herramienta electrónica para la recolección de datos. Los datos se analizaron mediante técnicas de estadística descriptiva. Resultados El 54,8% de los enfermeros utilizan tecnologías a través de los sistemas electrónicos SISREG y PEC, la calificación de la atención relacionada con la educación permanente en salud, cursos en línea y presenciales, además del uso de tecnologías de atención como la telesalud y la telemedicina. Conclusión Las acciones de gestión centradas en la tecnología mostraron un uso tímido. Las enfermeras deben empoderarse con estos dispositivos organizativos, con el fin de mejorar la atención y la calidad de la atención para el usuario.

Através da snowball technique, a amostra foi composta por 42 enfermeiros da Atenção Primária à Saúde, utilizando-se ferramenta eletrônica para coleta de dados. Os dados foram analisados por meio de técnicas de estatística descritiva. Resultados 54,8% dos enfermeiros utilizam tecnologias através de sistemas eletrônicos SISREG e PEC, da qualificação do cuidado relacionada a educação permanente em saúde, de cursos online e presenciais, além do uso de tecnologias do cuidado como o telessaúde e a telemedicina. Conclusão As ações de gerenciamento voltadas a tecnologia apresentaram utilização tímida. Os enfermeiros necessitam empoderar-se destes dispositivos organizacionais, tendo em vista o aprimoramento da assistência e qualidade do cuidado ao usuário.

          Equity of access to primary healthcare for vulnerable populations: the IMPACT international online survey of innovations

          Background Improving access to primary healthcare (PHC) for vulnerable populations is important for achieving health equity, yet this remains challenging. Evidence of effective interventions is rather limited and fragmented. We need to identify innovative ways to improve access to PHC for vulnerable populations, and to clarify which elements of health systems, organisations or services (supply-side dimensions of access) and abilities of patients or populations (demand-side dimensions of access) need to be strengthened to achieve transformative change. The work reported here was conducted as part of IMPACT (Innovative Models Promoting Access-to-Care Transformation), a 5-year Canadian-Australian research program aiming to identify, implement and trial best practice interventions to improve access to PHC for vulnerable populations. We undertook an environmental scan as a broad screening approach to identify the breadth of current innovations from the field. Methods We distributed a brief online survey to an international audience of PHC researchers, practitioners, policy makers and stakeholders using a combined email and social media approach. Respondents were invited to describe a program, service, approach or model of care that they considered innovative in helping vulnerable populations to get access to PHC. We used descriptive statistics to characterise the innovations and conducted a qualitative framework analysis to further examine the text describing each innovation. Results Seven hundred forty-four responses were recorded over a 6-week period. 240 unique examples of innovations originating from 14 countries were described, the majority from Canada and Australia. Most interventions targeted a diversity of population groups, were government funded and delivered in a community health, General Practice or outreach clinic setting. Interventions were mainly focused on the health sector and directed at organisational and/or system level determinants of access (supply-side). Few innovations were developed to enhance patients’ or populations’ abilities to access services (demand-side), and rarely did initiatives target both supply- and demand-side determinants of access. Conclusions A wide range of innovations improving access to PHC were identified. The access framework was useful in uncovering the disparity between supply- and demand-side dimensions and pinpointing areas which could benefit from further attention to close the equity gap for vulnerable populations in accessing PHC services that correspond to their needs.             The complexity of the work of nurses in Primary Health Care

            ABSTRACT Objective: to promote thinking on the work of nurses in Primary Health Care (PHC) and the necessary aspects for the (re)construction of this professional practice, reinforcing its role in the care of individuals, families and communities. Method: to apply the questions raised in the process of organizing a book, the literature and the PHS work experience of the authors. Results: Conflicts, dilemmas and relevant aspects of the practice of nurses in PHC are presented, contributing to critical thinking about the context of work and the need to articulate the category in the construction of its workspace. Final considerations: the practice of nurses in PHC is broad and a process of qualification field, whether performing in clinical, educational or managerial activities, and nurses need to be familiar with these contents in their daily work, seeking to articulate their class entities for the development of this specialty.

              The unpredictable journeys of spreading, sustaining and scaling healthcare innovations: a scoping review

              Innovation has the potential to improve the quality of care and health service delivery, but maximising the reach and impact of innovation to achieve large-scale health system transformation remains understudied. Interest is growing in three processes of the innovation journey within health systems, namely the spread, sustainability and scale-up (3S) of innovation. Recent reviews examine what we know about these processes. However, there is little research on how to support and operationalise the 3S. This study aims to improve our understanding of the 3S of healthcare innovations. We focus specifically on the definitions of the 3S, the mechanisms that underpin them, and the conditions that either enable or limit their potential. We conducted a scoping review, systematically investigating six bibliographic databases to search, screen and select relevant literature on the 3S of healthcare innovations. We screened 641 papers, then completed a full-text review of 112 identified as relevant based on title and abstract. A total of 24 papers were retained for analysis. Data were extracted and synthesised through descriptive and inductive thematic analysis. From this, we develop a framework of actionable guidance for health system actors aiming to leverage the 3S of innovation across five key areas of focus, as follows: (1) focus on the why, (2) focus on perceived-value and feasibility, (3) focus on what people do, rather than what they should be doing, (4) focus on creating a dialogue between policy and delivery, and (5) focus on inclusivity and capacity building. While there is no standardised approach to foster the 3S of healthcare innovations, a variety of practical frameworks and tools exist to support stakeholders along this journey.
