          Abstract

          Objetivo: determinar la prevalencia de sífilis gestacional e incidencia de sífilis congénita de la ciudad de Cali, Colombia en el año 2010. Materiales y métodos: estudio de corte transversal descriptivo. La población de estudio correspondió a las embarazadas que tuvieron control prenatal y asistieron para la atención de parto en Cali, y cuyo resultado materno y perinatal se conoció. Se excluyeron las pacientes con la ficha epidemiológica mal diligenciada. Se evaluaron las características sociodemográficas clínicas de las gestantes con sífilis gestacional, se estimó la prevalencia de sífilis gestacional e incidencia de sífilis congénita en la ciudad de Cali en el año 2010. Resultados: la prevalencia de sífilis gestacional encontrada fue de 1,4%, la incidencia de sífilis congénita para la ciudad de Cali fue del 2,5 por 1000 nacidos vivos en el año 2010. El 35% de las pacientes con serología positiva son tratadas inadecuadamente. Conclusiones: la prevalencia de sífilis gestacional es mayor a la esperada por la Organización Panamericana de la Salud.

          Translated abstract

          Objective: Obtaining gestational syphilis prevalence and congenital syphilis incidence data in the city of Cali, Colombia during 2010. Materials and methods: This was a descriptive, cross-sectional study. The study population consisted of pregnant women who had attended prenatal control sessions and given birth in Cali and whose maternal and perinatal results were known. Patients whose epidemiological data had not been properly recorded were excluded. The clinical sociodemographic characteristics of pregnant women suffering from gestational syphilis were determined. Gestational syphilis prevalence and congenital syphilis incidence in Cali during 2010 were estimated. Results: Gestational syphilis prevalence for Cali was 1.4% and congenital syphilis incidence was 2.5 per 1,000 live births during 2010; 35% of women having positive serology had not received suitable treatment. Conclusions: Gestational syphilis prevalence was greater than expected, in view of available PAHO data.

              Treatment of syphilis, 1989.

              With the introduction of penicillin after World War II, the incidence of syphilis in the United States decreased. Because of penicillin's great success, clinical trials stopped after an initial period of intensive investigation. Syphilis is a difficult disease to study; the natural history may span decades in an individual, and diagnosis and outcome are usually defined serologically, not clinically or bacteriologically. Although the recommended penicillin regimens changed, clinical trials were not repeated. Furthermore, because the early studies occurred before modern clinical-trial methodology was developed, interpretation of the results is difficult. As a result, while current regimens for syphilis therapy are effective, they may or may not be optimal. With the accumulation of reports of treatment failures and the recent appearance of human immunodeficiency virus, current regimens for the treatment of syphilis are being questioned. As background for a meeting at which treatment guidelines were reviewed, the available literature on syphilis therapy is summarized herein.
