          ABSTRACT

          Objective:

          to identify evidence about the use and effects of clinical simulation for preparing caregivers for discharging patients with chronic conditions.

          Methods:

          an integrative peer review in the Scopus, PubMed, Web of Science, Cumulative Index to Nursing and Allied Health Literature, ScienceDirect and Virtual Health Library databases, from July to September 2022.

          Results:

          3,218 studies were identified, with a final sample consisting of four national and two international articles. Using simulation as an educational technology contributed to caregiver preparation in home care. In most studies, using clinical simulation included using other strategies to complement training: expository dialogued class, conversation circle and audiovisual resources.

          Final considerations:

          simulation proved to be efficient for training caregivers, with the active participation of family members and nurses in health education actions.

          Objetivo:

          identificar evidências acerca do uso e dos efeitos da simulação clínica para o preparo de cuidadores na desospitalização de pacientes em condições crônicas.

          Métodos:

          revisão integrativa, realizada por pares, nas bases Scopus, PubMed, Web of Science, Cumulative Index to Nursing and Allied Health Literature, ScienceDirect e Biblioteca Virtual de Saúde, no período de julho a setembro de 2022.

          Resultados:

          foram identificados 3.218 estudos, com amostra final composta por quatro artigos nacionais e dois internacionais. O uso da simulação como tecnologia educacional contribuiu para o preparo de cuidadores nos cuidados domiciliares. Na maioria dos estudos, o uso da simulação clínica contou com a utilização de outras estratégias para complemento da formação: aula expositiva dialogada, roda de conversa e recursos audiovisuais.

          Considerações finais:

          a simulação se mostrou eficiente para o treinamento dos cuidadores, com participação ativa de familiares e enfermeiros nas ações de educação em saúde.

          Consideraciones finales:

          Integrative review: what is it? How to do it?

          ABSTRACT Introduction: The integrative review is the methodology that provides synthesis of knowledge and applicability of results of significant studies to practice. Objective: To present the phases of an integrative review and the relevant aspects to be taken into account when using this methodological resource. Methods: This study was based on bibliographic search and on the experience of the authors when performing an integrative review. Results: Presentation of the six stages of the integrative review process: preparing the guiding question, searching or sampling the literature, data collection, critical analysis of the studies included, discussion of results and presentation of the integrative review. Conclusions: Considering the need to assure care based on scientific evidence, the integrative review has been identified as a unique tool in healthcare for it synthesizes investigations available on the given topic and guides practice based on scientific knowledge.

            Defining and assessing professional competence.

            Current assessment formats for physicians and trainees reliably test core knowledge and basic skills. However, they may underemphasize some important domains of professional medical practice, including interpersonal skills, lifelong learning, professionalism, and integration of core knowledge into clinical practice. To propose a definition of professional competence, to review current means for assessing it, and to suggest new approaches to assessment. We searched the MEDLINE database from 1966 to 2001 and reference lists of relevant articles for English-language studies of reliability or validity of measures of competence of physicians, medical students, and residents. We excluded articles of a purely descriptive nature, duplicate reports, reviews, and opinions and position statements, which yielded 195 relevant citations. Data were abstracted by 1 of us (R.M.E.). Quality criteria for inclusion were broad, given the heterogeneity of interventions, complexity of outcome measures, and paucity of randomized or longitudinal study designs. We generated an inclusive definition of competence: the habitual and judicious use of communication, knowledge, technical skills, clinical reasoning, emotions, values, and reflection in daily practice for the benefit of the individual and the community being served. Aside from protecting the public and limiting access to advanced training, assessments should foster habits of learning and self-reflection and drive institutional change. Subjective, multiple-choice, and standardized patient assessments, although reliable, underemphasize important domains of professional competence: integration of knowledge and skills, context of care, information management, teamwork, health systems, and patient-physician relationships. Few assessments observe trainees in real-life situations, incorporate the perspectives of peers and patients, or use measures that predict clinical outcomes. In addition to assessments of basic skills, new formats that assess clinical reasoning, expert judgment, management of ambiguity, professionalism, time management, learning strategies, and teamwork promise a multidimensional assessment while maintaining adequate reliability and validity. Institutional support, reflection, and mentoring must accompany the development of assessment programs.
