      Long-term effect of the Brazilian Workers’ Food Program on the nutritional status of manufacturing workers: A population-based prospective cohort study

          Abstract

          Background

          The Brazilian Workers Food Program (WFP) is a public policy program of nutritional assistance to workers, with the main objective of improving nutritional conditions, which was implemented 40 years ago and serves over 21.4 million workers.

          Objectives

          To compare the long-term change in anthropometric indicators of the nutritional status and dietary intake between workers of manufacturing industries adherent to and non-adherent to the WFP.

          Methods

          A prospective cohort study, based on a combined stratified and multistage probability sampling, was carried out, with two waves with a 4-year interval. The change in body mass index (BMI), waist circumference (WC) and dietary intake at lunch by the 24-hour recall method were compared between groups with analysis of covariance.

          Results

          A total of 273 workers in 16 industries from an initial cohort of 1069 workers in 26 industries of the State of Rio Grande do Norte in Brazil were evaluated in the two waves. The mean age was 37±10 years and 53.1% were male, with no differences between groups in age and sex distribution. BMI increased in both groups (0.44 kg/m2 in non-WFP, p = 0.003, and 0.56 kg/m2 in WFP, p = 0.0006) and WC increased in the WFP group (1.50 cm, p = 0.0006). BMI change over time did not show statistical differences between groups (p = 0.54) but WC had a greater increase in the WFP group (difference 1.37 cm, p = 0.047). There were no differences between groups in the change over time of the dietary intake.

          Conclusion

          BMI and WC increased over time in manufacturing workers of industries both adherent and non-adherent to the WFP, but with a greater increase of WC in the WFP group. In order to achieve the objectives of the WFP, there will be a need for periodic evaluation and monitoring of nutritional indicators in these workers and implementation of monitoring and enforcement actions of the WFP.

            Development and testing of a past year measure of sedentary behavior: the SIT-Q

            Background Most sedentary behavior measures focus on occupational or leisure-time sitting. Our aim was to develop a comprehensive measure of adult sedentary behavior and establish its measurement properties. Method The SIT-Q was developed through expert review (n = 7), cognitive interviewing (n = 11) and pilot testing (n = 34). A convenience sample of 82 adults from Calgary, Alberta, Canada, participated in the measurement property study. Test-retest reliability was assessed by intraclass correlation coefficients (ICCs) comparing two administrations of the SIT-Q conducted one month apart. Convergent validity was established using Spearman’s rho, by comparing the SIT-Q estimates of sedentary behaviour with values derived from a 7-Day Activity Diary. Results The SIT-Q exhibited good face validity and acceptability during pilot testing. Within the measurement property study, the ICCs for test-retest reliability ranged from 0.31 for leisure-time computer use to 0.86 for occupational sitting. Total daily sitting demonstrated substantial correlation (ICC = 0.65, 95% CI: 0.49, 0.78). In terms of convergent validity, correlations varied from 0.19 for sitting during meals to 0.76 for occupational sitting. For total daily sitting, estimates derived from the SIT-Q and 7 Day Activity Diaries were moderately correlated (ρ = 0.53, p < 0.01). Conclusion The SIT-Q has acceptable measurement properties for use in epidemiologic studies.               Obesidade e políticas públicas: concepções e estratégias adotadas pelo governo brasileiro

              O estudo analisa estratégias nacionais de enfrentamento da obesidade no Brasil, no âmbito do Sistema Único de Saúde (SUS) e do Sistema Nacional de Segurança Alimentar e Nutricional (SISAN). Com base no método de análise documental, foram examinados documentos governamentais produzidos nos últimos 15 anos, nas seguintes dimensões: concepções de obesidade, ações propostas e estratégias de articulação entre setores. No âmbito do SUS, a obesidade é abordada como fator de risco e como doença, com enfoques individualizados e socioambientais, visando a alterar práticas alimentares e de atividade física. No SISAN, é concebida também como problema social, de insegurança alimentar e são propostos novos modos de produzir, comercializar e consumir alimentos para alterar as práticas alimentares de forma integrada. As propostas do SUS apontam para uma abordagem integrada e intrassetorial da obesidade, e as do SISAN reforçam a intersetorialidade em uma perspectiva ampliada que desafia as estruturas institucionais setoriais vigentes.
