          Abstract

          Resumo Objetivo: analisar a avaliação do crescimento e desenvolvimento infantil na consulta de enfermagem. Metodologia: estudo descritivo de abordagem qualitativa. Os participantes do estudo foram quatro enfermeiros que realizavam consulta de enfermagem à criança de maneira programática em unidades de saúde da família em Cuiabá, Mato Grosso, Brasil. A coleta dos dados foi realizada por meio da observação participante de 21 consultas de enfermagem, nos meses de janeiro e fevereiro de 2012. Os dados foram analisados pela técnica da análise de conteúdo. Resultados: com base na análise dos dados, foram identificadas duas categorias: Avaliação do crescimento infantil na consulta de enfermagem, e Avaliação do desenvolvimento infantil na consulta de enfermagem. Os enfermeiros avaliaram o crescimento infantil em todas as consultas, utilizando as curvas contidas na Caderneta de Saúde da Criança (CSC). No entanto, o desenvolvimento foi avaliado de forma parcial na maioria das consultas observadas. Conclusão: destacam-se como pontos positivos das consultas de enfermagem o fato dos enfermeiros avaliarem o crescimento e o desenvolvimento em todas as consultas e usarem a CSC como instrumento de orientação às mães e ferramenta para avaliação e acompanhamento do crescimento e desenvolvimento.

          Resumen Objetivo: analizar la evaluación del crecimiento y desarrollo infantil en la consulta de enfermería. Metodología: estudio descriptivo con enfoque cualitativo. Los participantes del estudio fueron cuatro enfermeros que realizaban la consulta de enfermería a los niños de manera programática en unidades de salud de familia en Cuiabá, Mato Grosso, Brasil. La recolección de los datos se llevó a cabo mediante la observación participante de 21 consultas de enfermería entre enero y febrero de 2012. Los datos se analizaron con la técnica de análisis de contenido. Resultados: a partir del análisis de los datos, surgieron las siguientes dos categorías: Evaluación del crecimiento infantil en la consulta de enfermería y Evaluación del desarrollo infantil en la consulta de enfermería. Los enfermeros evaluaron el crecimiento infantil en todas las consultas, utilizando las curvas contenidas en la libreta personal de registro médico del niño. Sin embargo, el desarrollo infantil sólo fue evaluado parcialmente en la mayoría de las consultas observadas. Conclusión: entre los aspectos más significativos de las consultas de enfermería se destacan la evaluación del crecimiento y desarrollo en todas ellas; y el uso de la libreta personal de registro médico del niño como instrumento de orientación para las madres y estrategia de seguimiento del crecimiento y desarrollo infantil.

          Translated abstract

          Abstract Objective: to analyze the child growth and development assessment in nursing consultation. Methodology: descriptive study with qualitative approach. The study involved four nurses who performed nursing consultation with children in a programmatic manner in family health units of Cuiabá, Mato Grosso, Brazil. Data were collected through participant observation of 21 nursing consultations from January to February 2012 and they were analyzed using the content analysis method. Results: on the basis of data analysis, emerged the following two categories: Child growth assessment in nursing consultation, and Child development assessment in nursing consultation. Nurses assessed the child growth in all of the consultations, using curves contained within the Personal Child Health Record (PCHR). Nevertheless, child development was assessed only in part in most consultations considered. Conclusion: the most significant features of the nursing consultations are the growth and development assessment in all of them, and the use of PCHR both as a guiding tool for mothers and a monitoring strategy for child growth and development.

          Survival, Morbidity, Growth and Developmental Delay for Babies Born Preterm in Low and Middle Income Countries – A Systematic Review of Outcomes Measured

          Background Premature birth is the leading cause of neonatal death and second leading in children under 5. Information on outcomes of preterm babies surviving the early neonatal period is sparse although it is considered a major determinant of immediate and long-term morbidity. Methods Systematic review of studies reporting outcomes for preterm babies in low and middle income settings was conducted using electronic databases, citation tracking, expert recommendations and “grey literature”. Reviewers screened titles, abstracts and articles. Data was extracted using inclusion and exclusion criteria, study site and facilities, assessment methods and outcomes of mortality, morbidity, growth and development. The Child Health Epidemiology Reference Group criteria (CHERG) were used to assess quality. Findings Of 197 eligible publications, few (10.7%) were high quality (CHERG). The majority (83.3%) report on the outcome of a sample of preterm babies at time of birth or admission. Only 16.0% studies report population-based data using standardised mortality definitions. In 50.5% of studies, gestational age assessment method was unclear. Only 15.8% followed-up infants for 2 years or more. Growth was reported using standardised definitions but recommended morbidity definitions were rarely used. The criteria for assessment of neurodevelopmental outcomes was variable with few standardised tools - Bayley II was used in approximately 33% of studies, few studies undertook sensory assessments. Conclusions To determine the relative contribution of preterm birth to the burden of disease in children and to inform the planning of healthcare interventions to address this burden, a renewed understanding of the assessment and documentation of outcomes for babies born preterm is needed. More studies assessing outcomes for preterm babies who survive the immediate newborn period are needed. More consistent use of data is vital with clear and aligned definitions of health outcomes in newborn (preterm or term) and intervention packages aimed to save lives and improve health.

            Support of breastfeeding by health professionals: integrative review of the literature

            Objective: To review the literature in order to evaluate how health professionals promote and support breastfeeding. Data sources: Studies from the following databases were retrieved: Scopus, PubMed, MEDLINE, Lilacs, SciELO, Web of Science and Cumulative Index to Nursing and Allied Health Literature (Cinahl). The descriptors “breastfeeding”, “professional role” and “patient care team” were used in the research. The review was limited to articles in Portuguese, Spanish, and English published between 1997 and 2013. Data synthesis: The search retrieved 1396 studies, 18 of which were selected for being directly relevant to the main question. The review showed that breastfeeding is a challenge for health professionals, regardless of their specialization, as they have to face a demand that requires skill and sensibility, for which they are not prepared. Health professionals have considered breastfeeding a purely instinctive and biological act. Moreover, it is noticeable that many of them possess theoretical expertise on the subject, but lack the practical skills. Conclusions: Health professionals need to be better trained to work on promoting breastfeeding, whether by health and medical schools or by healthcare administrators, in order to consolidate multiprofessional teams committed to maternal-infant health.

              Growth and development and their environmental and biological determinants

              Abstract Objective To investigate child growth, cognitive/language development, and their environmental and biological determinants. Methods This was a cross-sectional, predictive correlation study with all 92 children aged 24-36 months who attended the municipal early childhood education network in a town in the Vale do Jequitinhonha region, in 2011. The socioeconomic profile was determined using the questionnaire of the Associação Brasileira de Empresas de Pesquisa. The socio-demographicand maternal and child health profiles were created through a self-prepared questionnaire. The height-for-age indicator was selected to represent growth. Cognitive/language development was assessed through the Bayley Scale of Infant and Toddler Development. The quality of educational environments was assessed by Infant/Toddler Environment Scale; the home environment was assessed by the Home Observation for Measurement of the Environment. The neighborhood quality was determined by a self-prepared questionnaire. A multivariate linear regression analysis was performed. Results Families were predominantly from socioeconomic class D, with low parental education. The prevalence of stunted growth was 14.1%; cognitive and language development were below average at 28.6% and 28.3%, respectively. Educational institutions were classified as inadequate, and 69.6% of homes were classified as presenting a risk for development. Factors such as access to parks and pharmacies and perceived security received the worst score regarding neighborhood environment. Biological variables showed a greater association with growth and environmental variables with development. Conclusion The results showed a high prevalence of stunting and below-average results for cognitive/language development among the participating children. Both environmental and biological factors were related to growth and development. However, biological variables showed a greater association with growth, whereas environmental variables were associated with development.
