      Preterm Birth, Low Gestational Age, Low Birth Weight, Parity, and Other Determinants of Breech Presentation: Results from a Large Retrospective Population-Based Study

          Abstract

          Aim of this study is to analyze determinants of breech presentation using information from two regional databases of Lombardy (Italy) including data on consecutive singleton breech and vertex deliveries occurred in the Region, between January 2010 and December 2015. Breech presentation occurred in 3.8% of all single deliveries. Main determinants of breech presentation at birth were: gestational age and birth weight (the lower, the higher the incidence of breech presentation), maternal age (the older the mother, the higher the risk of breech presentation), parity (the frequency of breech decreased with increasing parity) and previous cesarean section. Breech presentation resulted more frequent after assisted reproduction procedures.

          Obstetric and perinatal outcomes in singleton pregnancies resulting from IVF/ICSI: a systematic review and meta-analysis.

          Earlier reviews have suggested that IVF/ICSI pregnancies are associated with higher risks. However, there have been recent advances in the way IVF/ICSI is done, leading to some controversy as to whether IVF/ICSI singletons are associated with higher perinatal risks. The objective of this systematic review was to provide an up-to-date comparison of obstetric and perinatal outcomes of the singletons born after IVF/ICSI and compare them with those of spontaneous conceptions. Extensive searches were done by two authors. The protocol was agreed a priori. PRISMA guidance was followed. The data were extracted in 2 × 2 tables. Risk ratio and risk difference were calculated on pooled data using Rev Man 5.1. Quality assessment of studies was performed using Critical Appraisal Skills programme. Sensitivity analysis was performed when the heterogeneity was high (I(2) > 50%). There were 20 matched cohort studies and 10 unmatched cohort studies included in this review. IVF/ICSI singleton pregnancies were associated with a higher risk (95% confidence interval) of ante-partum haemorrhage (2.49, 2.30-2.69), congenital anomalies (1.67, 1.33-2.09), hypertensive disorders of pregnancy (1.49, 1.39-1.59), preterm rupture of membranes (1.16, 1.07-1.26), Caesarean section (1.56, 1.51-1.60), low birthweight (1.65, 1.56-1.75), perinatal mortality (1.87, 1.48-2.37), preterm delivery (1.54, 1.47-1.62), gestational diabetes (1.48, 1.33-1.66), induction of labour (1.18, 1.10-1.28) and small for gestational age (1.39, 1.27-1.53). Singletons pregnancies after IVF/ICSI are associated with higher risks of obstetric and perinatal complications when compared with spontaneous conception. Further research is needed to determine which aspect of assisted reproduction technology poses most risk and how this risk can be minimized.

            Birth-weight-for-gestational-age patterns by race, sex, and parity in the United States population.

            To describe birth-weight-for-gestational-age patterns by race, sex, and parity in the United States population, and to discuss the measurements of gestational age by different methods, the pitfalls of each method, and the potential effects of the errors on birth-weight-for-gestational-age curves. We used the computerized certificates of live births from the entire population in 1989, consisting of more than four million infants born to residents of the United States. Gestational age was based on the date of the last menstrual period (LMP) modified by the clinical estimate in those situations in which normal distribution of birth weight does not apply. Birth weights for the tenth, 25th, 50th, 75th, and 90th percentiles were calculated by each gestational age and by race, sex, and parity. Eight sets of smoothed birth-weight-for-gestational-age curves were created for black-white, male-female, and primipara-multipara comparisons in sequence. Compared with previous major curves, our curves were closer to those in which the gestational age was derived from the LMP. There were marked differences observed between our curves and those in which the gestational age was based on ultrasound estimation. In the measurement of gestational age, the LMP may produce misclassification of gestational age, thereby elevating birth weight percentiles in preterm births and lowering birth weight percentiles in postterm births. However, ultrasound estimation is likely to create a differential misclassification of gestational age, which exerts the opposite effect of lowering birth weight percentiles early in gestation and increasing the percentiles late in gestation.

              New and old predictive factors for breech presentation: our experience in 14 433 singleton pregnancies and a literature review

              Breech presentation represents a common indication for primary cesarean section in women presenting for parturition. This study aims to investigate the presence of new and old risk factors for breech presentation and to provide a literature review.
