      Association between night work and dyslipidemia in South Korean men and women: a cross-sectional study

          Abstract

          Background

          Previous studies have reported that an irregular work schedule, particularly nighttime work, is associated with an altered lipid profile. Additionally, a mismatch in circadian rhythm can affect sleeping and eating habits, leading to poor health. This study aimed to examine the association between night work and dyslipidemia among South Korean adults aged ≥30 years.

          Methods

          For this study, the data of 5813 participants in the 2013–2016 Korea National Health and Nutrition Examination Survey were analyzed. Diagnoses of dyslipidemia were based on blood sampling tests of total cholesterol, high-density lipoprotein (HDL), low-density lipoprotein (LDL) cholesterol, and triglyceride levels. Night work was defined as that conducted during evening (6 P.M.–12 A.M.) and overnight hours (12 A.M.–8 A.M.). The association between night work hours and dyslipidemia in South Korean men and women was investigated using a logistic regression analysis.

          Results

          After adjusting for sociodemographic, economic, health-related, and nutritional factors, an association of night work with dyslipidemia was observed in male participants (odds ratio = 1.53, 95% confidence interval: 1.05–2.24). In subset analyses of male participants, night workers who skipped meals were more likely to have dyslipidemia than their day-working counterparts. Among men who slept < 7 h, night workers had a higher probability of dyslipidemia than day workers. In contrast, no statistically significant association between night work and dyslipidemia was observed in female participants, although the probability of dyslipidemia appeared to increase with advancing age. Furthermore, when women with dyslipidemia were subdivided by occupational categories, night workers in white collar positions were more likely to have dyslipidemia than their day-working counterparts.

          Conclusion

          Our study observed an association of night work with dyslipidemia, particularly in men. Although these findings may support interventions for South Korean night workers, further studies are needed for validation.

          Impacts of shift work on sleep and circadian rhythms.

          Shift work comprises work schedules that extend beyond the typical "nine-to-five" workday, wherein schedules often comprise early work start, compressed work weeks with 12-hour shifts, and night work. According to recent American and European surveys, between 15 and 30% of adult workers are engaged in some type of shift work, with 19% of the European population reportedly working at least 2 hours between 22:00 and 05:00. The 2005 International Classification of Sleep Disorders estimates that a shift work sleep disorder can be found in 2-5% of workers. This disorder is characterized by excessive sleepiness and/or sleep disruption for at least one month in relation with the atypical work schedule. Individual tolerance to shift work remains a complex problem that is affected by the number of consecutive work hours and shifts, the rest periods, and the predictability of work schedules. Sleepiness usually occurs during night shifts and is maximal at the end of the night. Impaired vigilance and performance occur around times of increased sleepiness and can seriously compromise workers' health and safety. Indeed, workers suffering from a shift work sleep-wake disorder can fall asleep involuntarily at work or while driving back home after a night shift. Working on atypical shifts has important socioeconomic impacts as it leads to an increased risk of accidents, workers' impairment and danger to public safety, especially at night. The aim of the present review is to review the circadian and sleep-wake disturbances associated with shift work as well as their medical impacts.

            2014 Clinical Practice Guidelines for Overweight and Obesity in Korea

            The dramatic increase in the prevalence of obesity and its accompanying comorbidities are major health concerns in Korea. Obesity is defined as a body mass index ≥25 kg/m2 in Korea. Current estimates are that 32.8% of adults are obese: 36.1% of men and 29.7% of women. The prevalence of being overweight and obese in national surveys is increasing steadily. Early detection and the proper management of obesity are urgently needed. Weight loss of 5% to 10% is the standard goal. In obese patients, control of cardiovascular risk factors deserves the same emphasis as weight-loss therapy. Since obesity is multifactorial, proper care of obesity requires a coordinated multidisciplinary treatment team, as a single intervention is unlikely to modify the incidence or natural history of obesity.

              Shift work, risk factors and cardiovascular disease.

              The literature on shift work, morbidity and mortality from cardiovascular disease, and changes in traditional risk factors is reviewed. Seventeen studies have dealt with shift work and cardiovascular disease risk. On balance, shift workers were found to have a 40% increase in risk. Causal mechanisms of this risk via known cardiovascular risk factors, in relation to circadian rhythms, disturbed sociotemporal patterns, social support, stress, behavior (smoking, diet, alcohol, exercise), and biochemical changes (cholesterol, triglycerides, etc) are discussed. The risk is probably multifactorial, but the literature has focused on the behavior of shift workers and has neglected other possible causal connections. In most studies methodological problems are present; these problems are related to selection bias, exposure classification, outcome classification, and the appropriateness of comparison groups. Suggestions for the direction of future research on this topic are proposed.
