          ABSTRACT

          OBJECTIVE:

          The primary objective of this study was to determine the prevalence of osteoporosis and osteopenia prior to total knee arthroplasty (TKA) in female patients. As a secondary objective, we evaluated the incidence of hip fractures, types of drugs to treat osteoporosis and serum vitamin D levels.

          METHOD:

          This is a transversal, descriptive and observational study which evaluated 60 women above age 55 prior to total knee replacement.

          RESULTS:

          Mean patient age was 71.4 years. Osteoporosis was present in 16.7% of the sample and osteopenia in 15%. In the patients with osteoporosis, femur fracture (20%) was most frequent. Most of the group with osteopenia did not take any medication to treat this condition (55.6%), while most patients with osteoporosis took alendronate (30%) and 30% did not take any medication.

          CONCLUSION:

          The female population awaiting total knee replacement should be considered at risk for osteoporosis, confirming recent findings in the literature. Level of Evidence III, Control Case Study.

          RESUMO

          OBJETIVO:

          O objetivo primário do estudo foi determinar a prevalência de osteoporose e osteopenia no pré-operatório de artroplastia total de joelho (ATJ) em pacientes do sexo feminino. Como objetivos secundários, avaliamos a incidência de fraturas de fêmur, o uso de medicações para o tratamento da osteoporose e os níveis da 25-OH vitamina D.

          MÉTODO:

          É um estudo transversal, descritivo e observacional. A amostra foi composta por 60 mulheres com idade acima de 55 anos, no climatério, em pré-operatório de ATJ.

          RESULTADOS:

          A média de idade foi de 71,4 anos. A osteoporose estava presente em 16,7% e a osteopenia em 15% da amostra estudada. Entre os pacientes com osteoporose, a fratura de fêmur foi a mais frequente (20%). A maioria do grupo com osteopenia não usava medicação para tratar essa afecção (55,6%), enquanto a maior parte dos pacientes com osteoporose usava alendronato (30%) e 30% não usavam nenhum medicamento.

          CONCLUSÃO:

          A população do sexo feminino aguardando artroplastia total de joelho deve ser considerada em risco de acometimento pela osteoporose, confirmando dados recentes da literatura. Nível de Evidência III, Estudo de Caso Controle.

          Aseptic loosening of total joint replacements: mechanisms underlying osteolysis and potential therapies

          Total joint replacement, although considered an excellent surgical procedure, can be complicated by osteolysis induced by implant particles and subsequent aseptic loosening of the implant. The pathogenesis of implant-associated osteolysis includes inflammatory and osteolytic processes. The sustained chronic inflammatory response initiated by particulate debris at the implant-bone interface is manifested by recruitment of a wide array of cell types. These cells include macrophages, fibroblasts, giant cells, neutrophils, lymphocytes, and – most importantly – osteoclasts, which are the principal bone resorbing cells. The 'cellular response' entails secretion of osteoclastogenic and inflammatory cytokines that favor exacerbated osteoclast activity and enhanced osteolysis. An appreciation of the complex network that leads to these cellular and inflammatory responses will form a foundation on which to develop therapeutic interventions to combat inflammatory periprosthetic bone loss.

            Clinical predictors of elective total joint replacement in persons with end-stage knee osteoarthritis

            Background Arthritis is a leading cause of disability in the United States. Total knee arthroplasty (TKA) has become the gold standard to manage the pain and disability associated with knee osteoarthritis (OA). Although more than 400 000 primary TKA surgeries are performed each year in the United States, not all individuals with knee OA elect to undergo the procedure. No clear consensus exists on criteria to determine who should undergo TKA. The purpose of this study was to determine which clinical factors will predict the decision to undergo TKA in individuals with end-stage knee OA. Knowledge of these factors will aid in clinical decision making for the timing of TKA. Methods Functional data from one hundred twenty persons with end-stage knee OA were obtained through a database. All of the individuals complained of knee pain during daily activities and had radiographic evidence of OA. Functional and clinical tests, collectively referred to as the Delaware Osteoarthritis Profile, were completed by a physical therapist. This profile consisted of measuring height, weight, quadriceps strength and active knee range of motion, while functional mobility was assessed using the Timed Up and Go (TUG) test and the Stair Climbing Task (SCT). Self-perceived functional ability was measured using the activities of daily living subscale of the Knee Outcome Survey (KOS-ADLS). A logistic regression model was used to identify variables predictive of TKA use. Results Forty subjects (33%) underwent TKA within two years of evaluation. These subjects were significantly older and had significantly slower TUG and SCT times (p < 0.05). Persons that underwent TKA were also significantly weaker, had lower self-reported function and had less knee extension than persons who did not undergo TKA. No differences between groups were seen for BMI, gender, knee flexion ROM and unilateral versus bilateral joint disease. Using backward regression, age, knee extension ROM and KOS-ADLS together significantly predicted whether or not a person would undergo TKA (p ≤ 0.001, R2 = 0.403). Conclusions Younger patients with full knee ROM who have a higher self-perception of function are less likely to undergo TKA. Physicians and clinicians should be aware that potentially modifiable factors, such as knee ROM can be addressed to potentially postpone the need for TKA.

              Análise da aplicabilidade de três instrumentos de avaliação de dor em distintas unidades de atendimento: ambulatório, enfermaria e urgência

              OBJETIVO: Avaliar a aplicabilidade dos instrumentos de avaliação de dor em três ambientes hospitalares. METODOLOGIA: Foram estudados 60 pacientes com dor musculoesquelética atendidos no Conjunto Hospitalar de Sorocaba: enfermaria da Ortopedia, ambulatório de Reumatologia e Unidade Regional de Emergência (Pronto-socorro). QUESTIONÁRIOS: Inventário breve de dor (BPI); questionário McGill de Dor (MPQ); escala visual analógica para dor (EVA). RESULTADOS: Na urgência, houve um predomínio do gênero masculino com média de idade de 35 anos. No ambulatório, foram entrevistados 18 homens com idade média de 42 anos e duas mulheres com idade média de 55 anos. Na enfermaria, predominavam os homens com idade média de 30,7 anos. No pronto-socorro e na enfermaria, a duração foi menor para a EVA e maior para o MPQ. A duração para a EVA foi menor e não diferiu com relação aos locais. Na enfermaria e no pronto-socorro, a preferência dos pacientes recaiu pelo BPI sendo que, na enfermaria, a EVA foi segunda opção. No ambulatório, a preferência dos pacientes recaiu sobre o BPI (80%) seguido do MPQ e os entrevistadores se dividiram igualmente entre esses mesmos questionários. No pronto-socorro, a preferência dos entrevistadores foi pelo BPI (40%), os restantes foram divididos igualmente. Houve uma prevalência maior de concordância do que de discordância das preferências entre pacientes e entrevistadores. CONCLUSÃO: Os instrumentos multidimensionais para avaliação da dor têm limitações em sua aplicabilidade no cotidiano da assistência hospitalar à saúde
