          Abstract

          Objetivo: Determinar a eficácia do laser de diodo e suas complicações no tratamento da retinopatia da prematuridade, estágio 3 limiar. Métodos: De 348 crianças pré-termo examinadas na Clínica Oftalmológica do Hospital Getúlio Vargas e Instituto de Olhos do Piauí, em Teresina, Piauí, no período de julho/89 a março/99, 152 (43,7%) apresentaram retinopatia da prematuridade. As crianças com retinopatia da prematuridade no estágio 3 limiar foram submetidas a laserablação retiniana com laser de diodo indireto, no centro cirúrgico, sob anestesia geral. Resultados: Vinte crianças (38 olhos) foram submetidas a laserablação retiniana. Doze pacientes (80,0%) tiveram regressão da retinopatia e três (20,0%) evoluíram para o estágio 5. Cinco crianças não retornaram para controle, sendo excluídas do estudo quanto aos resultados do laser. Não foram observadas complicações oculares. Cinco crianças apresentaram apnéia relacionada à anestesia. Conclusões: Neste grupo de crianças, o laser de diodo foi eficaz em 80,0% dos pacientes tratados. As complicações encontradas (cinco casos de apnéia) se relacionaram possivelmente à anestesia geral.

          Translated abstract

          Purpose: To determine the efficacy of the diode laser and its complications in treating retinopathy of prematurity, at stage 3, threshold. Methods: One hundred and fifty-two children, out of 348 premature infants examined at the Ophthalmologic Clinic of the Getúlio Vargas Hospital and Instituto de Olhos do Piauí, in Teresina - Piauí, from July '89 to March '99, had retinopathy of prematurity. Those with retinopathy of prematurity at stage 3, threshold, were submitted to application of diode indirect laser to the retina. Results: Twenty children (38 eyes) received diode laser. Twelve patients showed regression of retinopathy and three children developed stage 5. No ocular complications were observed. Five children had respiratory failure due to anesthesia. Conclusions: The diode laser treatment was successful in 80.0% of the patients. The complications were possibly related to the general anesthesia.

          An international classification of retinopathy of prematurity. The Committee for the Classification of Retinopathy of Prematurity.

          Because of modern life-support systems capable of keeping tiny premature infants alive, retinopathy of prematurity has recurred. No classification system currently available adequately describes the observations of the disease being made today. A new classification system, the work of 23 ophthalmologists from 11 countries, is presented in an attempt to meet this need. It emphasizes the location and the extent of the disease in the retina as well as its stages. The term "plus" is employed with the stage to denote progressive vascular incompetence. A computer-compatible diagram for recording the results of the examination employing the new classification system is furnished.

            Multicenter trial of cryotherapy for retinopathy of prematurity. Preliminary results. Cryotherapy for Retinopathy of Prematurity Cooperative Group.

            We report the preliminary three-month outcome of a multicenter randomized trial of cryotherapy for treatment of retinopathy of prematurity (ROP). Transscleral cryotherapy to the avascular retina was applied in one randomly selected eye when there was threshold disease (defined as five or more contiguous or eight cumulative 30 degrees sectors [clock hours] of stage 3 ROP in zone 1 or 2 in the presence of "plus" disease). An unfavorable outcome was defined as posterior retinal detachment, retinal fold involving the macula, or retrolental tissue. At this writing, 172 infants had been examined three months after randomization. An unfavorable outcome was significantly less frequent in the eyes undergoing cryotherapy (21.8%) compared with the untreated eyes (43%). While the surgery was stressful, no unexpected complications occurred during or following treatment. These data support the efficacy of cryotherapy in reducing by approximately one half the risk of unfavorable retinal outcome from threshold ROP.
