          Abstract

          BACKGROUND:

La belle indifference, a term that is inexhaustible to discuss, albeit there is no single definition that best describes the condition. To date, la belle indifference has gotten no place in the DSM-5 criteria for being a distinct entity of mental health issue.

          CASE REPORTS:

We reported 3 cases with la belle indifference. These patients include a stroke patient with right-sided hemiparesis, a patient suffering from neuropathic pain because of diabetic neuropathy, and a patient with cardiac dyspnea due to chronic heart failure — these three cases presented with a similar clinical profile that is the trigger for developing la belle indifference.

          CONCLUSIONS:

La belle indifference has varying symptom spectrum, and there is always something called “trigger” to induce it.

La belle indifférence in conversion symptoms and hysteria: systematic review.

La belle indifférence refers to an apparent lack of concern shown by some patients towards their symptoms. It is often regarded as typical of conversion symptoms/hysteria. To determine the frequency of la belle indifférence in studies of patients with conversion symptoms/hysteria and to determine whether it discriminates between conversion symptoms and symptoms attributable to organic disease. A systematic review of all studies published since 1965 that have reported rates of la belle indifférence in patients with conversion symptoms and/or patients with organic disease. A total of 11 studies were eligible for inclusion. The median frequency of la belle indifférence was 21% (range 0-54%) in 356 patients with conversion symptoms, and 29% (range 0-60%) in 157 patients with organic disease. The available evidence does not support the use of la belle indifférence to discriminate between conversion symptoms and symptoms of organic disease. The quality of the published studies is poor, with a lack off operational definitions and masked ratings. La belle indifférence should be abandoned as a clinical sign until both its definition and its utility have been clarified.

Uncovering the etiology of conversion disorder: insights from functional neuroimaging

Conversion disorder (CD) is a syndrome of neurological symptoms arising without organic cause, arguably in response to emotional stress, but the exact neural substrates of these symptoms and the underlying mechanisms remain poorly understood with the hunt for a biological basis afoot for centuries. In the past 15 years, novel insights have been gained with the advent of functional neuroimaging studies in patients suffering from CDs in both motor and nonmotor domains. This review summarizes recent functional neuroimaging studies including functional magnetic resonance imaging (fMRI), single photon emission computerized tomography (SPECT), and positron emission tomography (PET) to see whether they bring us closer to understanding the etiology of CD. Convergent functional neuroimaging findings suggest alterations in brain circuits that could point to different mechanisms for manifesting functional neurological symptoms, in contrast with feigning or healthy controls. Abnormalities in emotion processing and in emotion-motor processing suggest a diathesis, while differential reactions to certain stressors implicate a specific response to trauma. No comprehensive theory emerges from these clues, and all results remain preliminary, but functional neuroimaging has at least given grounds for hope that a model for CD may soon be found.
