          Abstract

          La apendicitis aguda es la causa más común de abdomen agudo que requiere tratamiento quirúrgico inmediato. A pesar de que muchos pacientes han recibido los beneficios de la apendicectomía videolaparoscópica, existen controversias respecto a su empleo sistemático en esta enfermedad. A tales efectos se realizó una revisión bibliográfica en la que se exponen algunos elementos actualizados sobre esta técnica, la cual, dada su utilidad diagnóstica y terapéutica, va ocupando progresivamente su lugar como proceder de elección en cualquier fase en que se encuentre el proceso morboso. Todo ello justifica la necesidad de profundizar en los principales aspectos cognoscitivos relacionados con este tema, pues solo así podrá elevarse la calidad asistencial en los hospitales y, con esa premisa, contar con protocolos de actuación uniformes, que beneficiarán a quienes presenten esta enfermedad tan frecuente

          Translated abstract

          Acute appendicitis is the most common cause of acute abdomen requiring immediate surgical treatment. Although many patients have received the benefits of the videolaparoscopic appendicectomy, there are controversies regarding its systematic use in this disease. To achieve this, a literature review was carried out in which some updated elements on this technique are exposed, which, given its diagnostic and therapeutic usefulness, is progressively occupying its place as election procedure in any phase during the morbid process. All these justifies the necessity of deepening in the main cognitive aspects related to this topic, because this is the way to rise the assistance quality in hospitals and, with that premise, to have uniform performance protocols that will benefit those who present this frequent disease

          Laparoscopic surgery and surgical infection.

          It has been observed that the metabolic response to surgical injury is less after laparoscopic surgery than after open surgery. However, the effect of laparoscopic surgery on surgical infection has not been given much attention in the surgical literature, even though it may decrease the incidence of infectious complications. The objective of this study was to assess the influence that laparoscopic surgery has on surgical infection and to highlight certain controversial aspects. A review of the literature was undertaken to examine the relationship between laparoscopic surgery and surgical infection. This was achieved primarily by using PubMed Medline as a source of material. Laparoscopic surgery is associated with better preservation of the immune system than open surgery. This results in a decreased incidence of infectious complications. Although carbon dioxide pneumoperitoneum affects the peritoneal response to injury, it seems to have no harmful effect in terms of intra-abdominal infection. Nevertheless, at laparoscopic operation the virulence of intestinal micro-organisms should be recognized and, while knowing the advantages of minimally invasive surgery, the surgeon should consider the complexity of this technique. Furthermore, maintenance of laparoscopic instruments should be governed by the same norms as those used in open surgery; recommendations offered by the manufacturers should be respected.

            Complicaciones de la apendicectomía por apendicitis aguda

            INTRODUCCIÓN. La apendicectomía por apendicitis aguda es la operación de urgencia más común en los servicios quirúrgicos, pero no está exenta de complicaciones condicionadas por factores poco conocidos, cuya determinación podría disminuir la morbilidad y mortalidad por esta causa. Fue objetivo de esta investigación identificar algunos factores relacionados con la aparición de complicaciones en los pacientes apendicectomizados por apendicitis aguda. MÉTODOS. Se realizó un estudio descriptivo, observacional y prospectivo de 560 pacientes apendicectomizados, con diagnóstico histopatológico de apendicitis aguda, egresados del Servicio de Cirugía General del Hospital Provincial Docente «Saturnino Lora» de Santiago de Cuba durante el 2006. RESULTADOS. El 21,6 % de la serie sufrió algún tipo de complicación, principalmente la infección del sitio operatorio. Las complicaciones aumentaron en relación con la edad y se presentaron más frecuentemente en los pacientes con enfermedades asociadas, estado físico más precario, mayor tiempo de evolución preoperatoria, así como en las formas histopatológicas más avanzadas de la afección (en las que se incluyen los 4 pacientes fallecidos). La aparición de tales complicaciones puede ser causa de reintervenciones y de aumento de la estadía hospitalaria. CONCLUSIONES. El diagnóstico precoz de la enfermedad y la apendicectomía inmediata con una técnica quirúrgica adecuada previenen la aparición de complicaciones posquirúrgicas y determinan el éxito del único tratamiento eficaz contra la afección más común que causa el abdomen agudo, cuyo pronóstico depende en gran medida y entre otros factores, del tiempo de evolución preoperatoria y de la fase en que se encuentre el proceso morboso al realizar la intervención.

              Laparoscopic appendectomy for complicated appendicitis: an evaluation of postoperative factors.

              The use of laparoscopic appendectomy for complicated appendicitis is controversial. Outcomes were compared between patients who had complicated appendicitis and those who had uncomplicated appendicitis. Consecutive patients (n = 304) who underwent laparoscopic appendectomy were studied. Patients undergoing open appendectomies also were compared ad hoc. Analgesia use, length of hospital stay, return to activity, and complication rates for the complicated and uncomplicated appendicitis subgroups were analyzed. Complete data were available for 243 patients (80%). There were no statistical differences in characteristics between the two groups. The operating times, lengths of hospital stay, return to activity times, complication rates, and analgesia requirements, both in the hospital and after discharge, were equivalent. A greater number of complicated cases required open conversion. Considering those with complicated appendicitis, the open group had a significantly longer mean hospital stay and a higher complication rate than those treated with laparoscopic appendectomy. The minimally invasive laparoscopic technique is safe and efficacious. It should be the initial procedure of choice for most cases of complicated appendicitis.
