      Epidemiological profile of leishmaniasis at a reference service in the state of Alagoas, Brazil, from january 2000 to september 2008

          Abstract

          Leishmaniasis is a parasitic disease found in the continents of Europe, Asia, Africa and the Americas. In Brazil, the disease is an important public health problem, occurring in most states, but mainly in the Northeast region of the country, with the state of Alagoas having a significant number of confirmed cases. The present study establishes and discusses the epidemiological profile of cases of leishmaniases treated at the Reference Center of Alagoas, Hospital Escola Hélvio de Farias Auto, between January 2000 and October 2008. During thisperiod, records of 2,104 patients notified to the hospital Epidemiological Surveillance Center were reviewed. The epidemiology of leishmaniasis, both visceral and tegumentary, has shown a profile that has changed over throughout the years, with a decrease in the number of notified cases. Disease occurrence was associated with lower social economic classes, and affected mainly rural workers in general, housewives and students.

          Clinical and immunopathological spectrum of American cutaneous leishmaniasis with special reference to the disease in Amazonian Brazil: a review

          The wide variety of Leishmania species responsible for human American cutaneous leishmaniasis combined with the immune mechanisms of the host results in a large spectrum of clinical, histopathological, and immunopathological manifestations. At the middle of this spectrum are the most frequent cases of localized cutaneous leishmaniasis (LCL) caused by members of the subgenera Leishmania and Viannia, which respond well to conventional therapy. The two pathogenicity extremes of the spectrum generally recognized are represented at the hypersensitivity pole by mucocutaneous leishmaniasis (MCL) and at the hyposensitivity pole by anergic diffuse cutaneous leishmaniasis (ADCL). Following the present study on the clinical, histopathological and immunopathological features of cutaneous leishmaniasis in Amazonian Brazil, we propose the use of the term "borderline disseminated cutaneous leishmaniasis" for the disseminated form of the disease, due to parasites of the subgenera Leishmania and Viannia, which might be regarded as intermediate between LCL and the extreme pathogenicity poles MCL and ADCL.

              Epidemiologia da leishmaniose tegumentar na Comunidade São João, Manaus, Amazonas, Brasil

              Em Manaus, Amazonas, Brasil, o grau de exposição dos indivíduos à leishmaniose está relacionado aos processos de ocupação desordenada. Para avaliar fatores predisponentes à gênese do surto, confirmar o diagnóstico parasitológico, tratar os doentes, avaliar agentes etiológicos, reservatórios e transmissores, realizou-se um estudo em Manaus, numa comunidade localizada no Km 4 da BR-174, durante 12 meses. Atendeu-se 451 indivíduos, dos quais foram diagnosticados 17 casos de leishmaniose tegumentar americana. Seis pacientes eram mulheres e 11 homens. A idade variou de um a 64 anos. Onze pacientes tinham entre uma e três lesões. Em relação aos reservatórios, três marsupiais foram capturados. Não foram encontrados hemoflagelados nos exames realizados. Dentre os insetos capturados, Lutzomyia umbratilis foi a espécie predominante. Grande parte dos pacientes de leishmaniose tegumentar americana tem atividades que os expõem aos vetores da Leishmania. Um grupo de pacientes foi infectado possivelmente no intra/peridomicílio. O perfil epidemiológico da leishmaniose tegumentar americana nessa comunidade é semelhante àquele observado em outros focos na região. Com essa casuística, a leishmaniose tegumentar americana é caracterizada como endêmica e como um problema de saúde pública local.
