          Abstract

          OBJETIVOS: Comparar a ultrassonometria do calcâneo com a densitometria óssea na avaliação de fraturas vertebrais morfométricas em homens acima de 60 anos. SUJEITOS E MÉTODOS: Foram analisados 96 homens com mais de 60 anos por meio de densitometria óssea da coluna, fêmur e rádio, radiografia lateral da coluna torácica e lombar e ultrassonometria do calcâneo. RESULTADOS: Cinquenta e um por cento dos homens apresentaram osteoporose e fraturas vertebrais. Foi observada correlação entre índice de stiffness e T-score da ultrassonometria e as densidades minerais ósseas (DMO) de todos os sítios. Quanto à presença de fraturas, identificou-se correlação com a DMO do rádio ultradistal e 33%. Por meio da curva ROC, observou-se acurácia da DMO do rádio UD na detecção de fraturas vertebrais. CONCLUSÕES: Nosso estudo mostrou correlação entre a ultrassonometria e a densitometria no diagnóstico de osteoporose em homens acima dos 60 anos. Também se identificou correlação entre fratura vertebral morfométrica e a DMO do rádio.

          Translated abstract

          OBJECTIVES: To compare calcaneal ultrasonometry and bone densitometry in the evaluation of morphometric vertebral fractures in men over 60 years of age. SUBJECTS AND METHODS: We studied 96 men over 60 years of age by means of bone densitometry of the spine, femur and radius, lateral radiograph of the thoracic and lumbar spine, and calcaneal ultrasonometry. RESULTS: Fifty-one percent of men had osteoporosis and vertebral fractures. Correlation was found between ultrasonometry stiffness index, T-score and bone mineral density of the spine, femur and radius (p < 0.01). Regarding the presence of fractures, there was a correlation only with BMD of the ultradistal radius (UD) and radius 33%. ROC curve showed accuracy only of UD radius BMD in detecting vertebral fractures. CONCLUSIONS: Our study showed a correlation between osteoporosis diagnosis by ultrasonometry and densitometry in men over 60 years. It also showed a correlation between morphometric vertebral fracture and bone mineral density of the forearm.

          Vertebral fracture assessment using a semiquantitative technique.

          The assessment of vertebral fracture by conventional radiography has been refined and improved using either semiquantitative or quantitative criteria. The inter- and intraobserver variability was determined for a semiquantitative visual approach that we routinely use in clinical studies for assessing prevalent and incident vertebral fractures. In addition, the semiquantitative approach was compared with a quantitative morphometric approach. The incidence and prevalence of vertebral fractures were determined in 57 postmenopausal women (age 65-75 years) by three independent observers. The radiographic basis for fracture definitions and the source of interobserver agreement for the semiquantitative technique. We conclude that the semiquantitative approach can be applied reliably in vertebral fracture assessment when performed using well-defined criteria.

              The prevalence of vertebral deformity in european men and women: the European Vertebral Osteoporosis Study.

              Our aim was to determine the prevalence of radiographically defined vertebral deformity, as a marker of vertebral osteoporosis, in different regions and populations within Europe. We used a cross-sectional population-based survey. Population-based sampling frames were obtained from 36 centers in 19 European countries. Stratified random sampling was used to recruit 15,570 males and females aged 50-79 years. Lateral spinal radiographs were taken according to a standardized protocol, and all X-rays were evaluated centrally. Vertebral deformity was morphometrically defined according to the published methods of McCloskey and Eastell. Based on the McCloskey method, the mean center prevalence of all deformities was 12% in females (range 6-21%) and 12% in males (range 8-20%). The prevalence increased with age in both sexes though the gradient was steeper in females. There was substantial geographical variation, with the highest rates in Scandinavian countries. Radiographically defined vertebral deformity is a common disorder and equally frequent in males and females. Using standardized methodology, there is important variation in occurrence across Europe, which might suggest clues to pathogenesis.
