      A within-subject comparison of the conventional clasp-retained with attachment-retained removable partial dentures

          Abstract

          Objective

          This study conducted to satisfaction with conventional clasp-retained and attachment-retained removable partial dentures (RPDs) among patients with partially edentulous maxilla.

          Methods

          The crossover trial recruited 10 patients with bilateral free-end partially edentulous maxilla who received a conventional RPD for 3 months, followed by an attachment-retained removable partial denture (ARRPD) for another 3 months. There was no washout period between the two interventions. During follow-up, patients were requested to complete a patient satisfaction questionnaire. This 9-item validated questionnaire measured patient satisfaction with the ease of cleaning, ability to speak, comfort, aesthetics, stability, ability to masticate different types of food, masticatory efficiency, oral condition, and general satisfaction.

          Results

          The comparison of the two treatment modalities showed significantly higher satisfaction with the ARRPD than with the conventional RPD. The ARRPD was preferred due to the ease to clean, speech, comfort, aesthetics, stability, masticatory ability, and masticatory efficiency ( p < .05).

          Conclusion

          The study showed higher short-term satisfaction rates in patients with ARRPDs than with the conventional clasp-retained RPDs. The superior aesthetics of ARRPDs are recognized in conjunction with the restoration of the partially edentulous maxilla.

          أهداف البحث

          تقييم رضا المرضى عن أطقم الأسنان الجزئية بالمشابك التقليدية مقابل تلك التي تعتمد على الثبات عن طريق الملحقات الدقيقة عند إعادة تأهيل الفك العلوي.

          طرق البحث

          تتضمن هذه الدراسة ١٠ مرضى يعانون من فقدان جزئي للأسنان العلوية تم تعويضها بتركيبات جزئية متحركة بمشابك تقليدية، وتم متابعتها على مدى ثلاثة أشهر من ثم تم استبدالها بتركيبات أخرى تعتمد على الملحقات الدقيقة، وتم متابعتها لثلاثة أشهر أخرى من دون عمل فترة راحة بين التركيبتين. خلال هذه الفترة طلب من المرضي تعبئة استبانة عن رضائهم عن التركيبات، وتشمل الاستبانة أسئلة تشمل سهولة التنظيف، والمقدرة على الكلام، والراحة، والجمالية، والثبات، والمقدرة على المضغ، وكفاءة المضغ، والرضا العام.

          النتائج

          أظهرت النتائج رضا أكثر للمرضى بالتركيبات المتحركة بالملحقات الدقيقة مقارنة بتلك المعتمدة على المشابك التقليدية، بسبب سهولة التنظيف، والمقدرة على الكلام، والراحة، والجمالية، والثبات، والمقدرة على المضغ، وكفاءة المضغ.

          الاستنتاجات

          تمثل التركيبات المتحركة المعتمده على الملحقات الدقيقة بديل جيد عن تلك التي تعتمد على المشابك التقليدية خاصة عندما يتم تعويض الأسنان في الفك العلوي.

          The validation of the visual analogue scale for patient satisfaction after total hip arthroplasty

          Introduction Patient satisfaction becomes more important in our modern health care system. The assessment of satisfaction is difficult because it is a multifactorial item for which no golden standard exists. One of the potential methods of measuring satisfaction is by using the well-known visual analogue scale (VAS). In this study, we validated VAS for satisfaction. Patient and methods In this prospective study, we studied 147 patients (153 hips). The construct validity was measured using the Spearman correlation test that compares the satisfaction VAS with the Harris hip score, pain VAS at rest and during activity, Oxford hip score, Short Form 36 and Western Ontario McMaster Universities Osteoarthritis Index. The reliability was tested using the intra-class coefficient. Results The Pearson correlation test showed correlations in the range of 0.40–0.80. The satisfaction VAS had a high correlation between the pain VAS and Oxford hip score, which could mean that pain is one of the most important factors in patient satisfaction. The intra-class coefficient was 0.95. Conclusions There is a moderate to mark degree of correlation between the satisfaction VAS and the currently available subjective and objective scoring systems. The intra-class coefficient of 0.95 indicates an excellent test–retest reliability. The VAS satisfaction is a simple instrument to quantify the satisfaction of a patient after total hip arthroplasty. In this study, we showed that the satisfaction VAS has a good validity and reliability.

            Measuring patient satisfaction with mandibular prostheses.

            Previous research has shown that patients' evaluations of their prostheses correlate poorly with the clinicians' assessments, as well as with intraoral anatomic factors. It has been recommended that researchers conduct more studies that use patient satisfaction as the primary outcome measure in treatment evaluation and that more attention be paid to understanding exactly what measures of patient satisfaction represent. In this study, the relationship between patients' ratings of general satisfaction and their perceptions of different aspects of mandibular prostheses is investigated. One hundred and twenty subjects applied to participate in a randomized controlled clinical trial comparing two types of mandibular prostheses: conventional dentures and implant prostheses. At baseline, they were asked to rate on 100 mm visual analog scales (VAS) factors that edentulous patients indicated were important to them. These included comfort, ability to chew, stability, esthetics, ability to speak and ease of cleaning of their conventional dentures. Subjects were also asked to rate their general satisfaction with their dentures. In addition, they selected the one quality of their denture that they considered to be most important. Multiple regression methods revealed that gender, as well as patients' ratings of comfort, stability, esthetics, ability to chew and ability to speak with their prostheses contributed significantly to general satisfaction (F<0.0001). Furthermore, 89% of the variation in ratings of general satisfaction was explained by these factors. In addition, patients who considered ability to chew as the most important factor associated with their dentures rated their general satisfaction significantly higher than the other subjects (P=0.0003). Patient satisfaction with conventional dental prostheses is highly dependent on gender, and the appearance and functionality of the appliance. The combined effect of these factors explained most of the variation in the satisfaction ratings.

              Within-subject comparisons of implant-supported mandibular prostheses: psychometric evaluation.

              In a within-subject cross-over clinical trial, psychometric and functional measurements were taken while 15 completely edentulous subjects wore mandibular fixed prostheses and long-bar removable implant-supported prostheses. In this paper, the results of a psychometric assessment are presented. Eight subjects first received the fixed bridge and seven the removable type. After having worn a prosthesis for a minimum of two months, subjects responded to psychometric scales that measured their perceptions of various factors associated with prostheses. They also chewed test foods while masticatory activity was recorded. The prostheses were then changed and the procedures repeated. At the end of the study, patients were asked to choose the prosthesis that they wished to keep. Patients assigned significantly higher scores, on visual analogue scales, to both types of implant-supported prostheses than to their original conventional prostheses for all factors tested, including general satisfaction. However, no statistically significant differences between the two implant-supported prostheses were detected except for the difficulty of chewing carrot, apple, and sausage. For these foods, the fixed prostheses were rated higher. Subjects' responses to category scales were consistent with their responses to the visual analogue scales. These results suggest that, although patients find the fixed bridge to be significantly better for chewing harder foods, there is no difference in their general satisfaction with the two types of prostheses.
