      A systematic review of school meal nudge interventions to improve youth food behaviors

          Abstract

          Background

          School meal programs have a large reach and thus are ideal environments in which to implement interventions targeting improved youth eating behaviors and reduced food waste. This systematic review summarizes the evidence on the effectiveness of school meal nudge interventions on influencing children’s eating and waste behaviors.

          Methods

          Inclusion criteria required studies have participants in primary or secondary school (grades K-12) with interventions that occurred during school lunch or breakfast in the cafeteria and included at least one of the following outcomes: selection, consumption, waste, or school meal participation. Analyses of intervention outcomes were restricted to studies of strong and moderate quality.

          Results

          Twenty-nine studies were included in the quality assessment. Included interventions fell into three categories: 1) placement/convenience, 2) marketing/promotion, or 3) variety/portions. The 20 strong and moderate quality studies included in outcome analyses generally used strong data collection methods and study designs, but were limited by an overall lack of intervention fidelity checks. Multi-component interventions often did not use methods that allowed for separate analyses of outcomes for different intervention components.

          Conclusions

          School meal nudge interventions were positively associated with food selection, and had an inconsistent relationship with food consumption. There were few studies evaluating the impact of nudge interventions on meal participation or food waste. The limited evidence available links nudges to improved meal participation, as well as undesirable increases in food waste. Future research in this area should use methods that incorporate implementation metrics, attend to systems factors, and allow the outcomes of individual intervention components to be isolated.

          Social modeling of eating: a review of when and why social influence affects food intake and choice.

          A major determinant of human eating behavior is social modeling, whereby people use others' eating as a guide for what and how much to eat. We review the experimental studies that have independently manipulated the eating behavior of a social referent (either through a live confederate or remotely) and measured either food choice or intake. Sixty-nine eligible experiments (with over 5800 participants) were identified that were published between 1974 and 2014. Speaking to the robustness of the modeling phenomenon, 64 of these studies have found a statistically significant modeling effect, despite substantial diversity in methodology, food type, social context and participant demographics. In reviewing the key findings from these studies, we conclude that there is limited evidence for a moderating effect of hunger, personality, age, weight or the presence of others (i.e., where the confederate is live vs. remote). There is inconclusive evidence for whether sex, attention, impulsivity and eating goals moderate modeling, and for whether modeling of food choice is as strong as modeling of food intake. Effects with substantial evidence were: modeling is increased when individuals desire to affiliate with the model, or perceive themselves to be similar to the model; modeling is attenuated (but still significant) for healthy-snack foods and meals such as breakfast and lunch, and modeling is at least partially mediated through behavioral mimicry, which occurs without conscious awareness. We discuss evidence suggesting that modeling is motivated by goals of both affiliation and uncertainty-reduction, and outline how these might be theoretically integrated. Finally, we argue for the importance of taking modeling beyond the laboratory and bringing it to bear on the important societal challenges of obesity and disordered eating.

            Social norms and their influence on eating behaviours.

            Social norms are implicit codes of conduct that provide a guide to appropriate action. There is ample evidence that social norms about eating have a powerful effect on both food choice and amounts consumed. This review explores the reasons why people follow social eating norms and the factors that moderate norm following. It is proposed that eating norms are followed because they provide information about safe foods and facilitate food sharing. Norms are a powerful influence on behaviour because following (or not following) norms is associated with social judgements. Norm following is more likely when there is uncertainty about what constitutes correct behaviour and when there is greater shared identity with the norm referent group. Social norms may affect food choice and intake by altering self-perceptions and/or by altering the sensory/hedonic evaluation of foods. The same neural systems that mediate the rewarding effects of food itself are likely to reinforce the following of eating norms.

              Influence of peers and friends on children's and adolescents' eating and activity behaviors.

              Obesity during childhood and adolescence is a growing problem in the United States, Canada, and around the world that leads to significant physical, psychological, and social consequences. Peer experiences have been theoretically and empirically related to the "Big Two" contributors to the obesity epidemic, unhealthy eating and physical inactivity. In this article, we synthesize the empirical literature on the influence of peers and friends on youth's eating and physical activity. Limitations and issues in the theoretical and empirical literatures are also discussed, along with future research directions. In conclusion, we argue that the involvement of children's and adolescents' peer networks in prevention and intervention efforts may be critical for promoting and maintaining positive behavioral health trajectories. However, further theoretical and empirical work is needed to better understand the specific mechanisms underlying the effects of peers on youth's eating and physical activity.
