          Abstract

          Introducción. La hipersensibilidad o anafilaxia durante el período perioperatorio casi siempre se transforma en un enigma diagnóstico. En primer término es una complicación grave para el anestesiólogo y máxime si esta ocurre en un paciente tan altamente sensible como es la materna. Objetivos: Exponer el caso clínico de una paciente obstétrica con un embarazo del primer trimestre que sufrió una respuesta anafiláctica a fármacos anestésicos utilizados. Presentación del caso: Se trata de una paciente de 28 años de edad, con antecedente de asma bronquial intermitente desde su niñez, que se automedicaba con spray de salbutamol pues sus crisis eran ocasionales y solo necesitó ser tratada con corticoides en una ocasión en los últimos 5 años. No encontramos otro antecedente de relevancia. Presentaba embarazo del primer trimestre. Llegó al quirófano con diagnóstico de quiste de ovario torcido como emergencia quirúrgica y sufrió una respuesta anafiláctica a fármacos anestésicos utilizados en la inducción anestésica y sufrió colapso vascular y paro cardiorrespiratorio como respuesta anafiláctica a fármacos anestésicos utilizados, logrando resolver este grave cuadro sintomático y salvar la vida de de la gestante sin secuelas. Conclusiones: Las reacciones anafilácticas o seudoanafilácticas, constituyen una preocupación para el anestesiólogo, por lo que es imprescindible indagar antecedentes en la evaluación preanestésica aún en procederes quirúrgicos de urgencia.

          Translated abstract

          Introduction: The hypersensitivity or anaphylaxis during the perioperative period most of the times becomes a diagnostic enigma. First, it is severe complication for the anesthesiologist and especially if it occurs in as sensitive as the mother one. Objectives: To show the clinical case of a obstetric patient with a first trimester pregnancy with a anaphylactic response to the anesthetic drugs used. Case presentation: This is patient aged 2l8 with a history of intermittent bronchial asthma from childhood who self-medicate with Salbutamol spray since its crises were occasional and only needed to be treated sometimes with corticoids over the past 5 years. There was not another significant background. The came to operating theatre with a diagnosis of a twisted ovarian cyst as a surgical emergence and had a anaphylactic response to the anesthetic drugs used in the anesthetic induction a suffered a vascular collapse and cardiac-respiratory arrest as a anaphylactic response to anesthetic drugs used with solution of this severe symptomatic picture saving its life without sequelae. Conclusions: The anaphylactic or pseudo-anaphylactic reactions are a concern for anesthesiologist, thus it is essential to look for the backgrounds in pre-anesthetic assessment even in emergence surgical procedures.
