          Abstract

          Mediante diversos experimentos se ha comprobado que la oxigenación hiperbárica favorece el proceso cicatrizal, evitando los retardos de consolidación y cicatrización. El objetivo fue describir la evolución de un paciente con fractura traumática expuesta, luego de la aplicación de oxigenación hiperbárica. Paciente masculino de 51 años de edad, que sufrió un accidente laboral ocasionándole fractura expuesta del tercio medio distal de cúbito y radio del antebrazo izquierdo, así como sección de músculos y vasos sanguíneos, provocándole una atrofia ósea aguda de Sudeck. Durante la evolución del paciente, después del tratamiento quirúrgico se le indicó oxigenación hiperbárica por un período de 5 meses en tres ciclos de 20 sesiones cada uno, durante el tercer ciclo se comenzó a aplicar fisioterapia y rehabilitación. Como resultado del tratamiento con oxigenación hiperbárica, fue efectivo en la disminución del tiempo de evolución de las lesiones traumáticas, lográndose mejorar la sintomatología y el estado funcional del miembro afectado en el paciente.

          Translated abstract

          Through diverse experiments it has been proved that the hyperbaric oxygenation facilitates scarring process avoiding consolidation and healing delays. The objective was describing the evolution of a patient with exposed traumatic fracture after applying hyperbaric oxygen treatment. Male patient aged 51 years who suffered a work accident causing a middle distal third exposed fracture of the left forearm ulna and radius as well as muscles and blood vessels section, and provoking a sharp bone Sudeck atrophy. After the surgical treatment, during the patient`s evolution, hyperbaric oxygenation was indicated for a period of 5 months in three cycles of 20 sessions each; during the third cycle physical therapy and rehabilitation began to be applied. Hyperbaric oxygenation treatment was effective, shortening traumatic lesions evolution time and improving symptoms and the patient injured member functional status.

          A phase I study of low-pressure hyperbaric oxygen therapy for blast-induced post-concussion syndrome and post-traumatic stress disorder.

          This is a preliminary report on the safety and efficacy of 1.5 ATA hyperbaric oxygen therapy (HBOT) in military subjects with chronic blast-induced mild to moderate traumatic brain injury (TBI)/post-concussion syndrome (PCS) and post-traumatic stress disorder (PTSD). Sixteen military subjects received 40 1.5 ATA/60 min HBOT sessions in 30 days. Symptoms, physical and neurological exams, SPECT brain imaging, and neuropsychological and psychological testing were completed before and within 1 week after treatment. Subjects experienced reversible middle ear barotrauma (5), transient deterioration in symptoms (4), and reversible bronchospasm (1); one subject withdrew. Post-treatment testing demonstrated significant improvement in: symptoms, neurological exam, full-scale IQ (+14.8 points; p<0.001), WMS IV Delayed Memory (p=0.026), WMS-IV Working Memory (p=0.003), Stroop Test (p<0.001), TOVA Impulsivity (p=0.041), TOVA Variability (p=0.045), Grooved Pegboard (p=0.028), PCS symptoms (Rivermead PCSQ: p=0.0002), PTSD symptoms (PCL-M: p<0.001), depression (PHQ-9: p<0.001), anxiety (GAD-7: p=0.007), quality of life (MPQoL: p=0.003), and self-report of percent of normal (p<0.001), SPECT coefficient of variation in all white matter and some gray matter ROIs after the first HBOT, and in half of white matter ROIs after 40 HBOT sessions, and SPECT statistical parametric mapping analysis (diffuse improvements in regional cerebral blood flow after 1 and 40 HBOT sessions). Forty 1.5 ATA HBOT sessions in 1 month was safe in a military cohort with chronic blast-induced PCS and PTSD. Significant improvements occurred in symptoms, abnormal physical exam findings, cognitive testing, and quality-of-life measurements, with concomitant significant improvements in SPECT.

            Hyperbaric oxygen therapy in necrotizing soft tissue infections.

            Surgical debridement and antibiotics are the mainstays of therapy for patients with necrotizing soft tissue infections (NSTIs), but hyperbaric oxygen therapy (HBO) is often used as an adjunctive measure. Despite this, the efficacy of HBO remains unclear. We hypothesized that HBO would have no effect on mortality or amputation rates.

              Evaluation of the effects of hyperbaric oxygen therapy for spinal cord lesion in correlation with the moment of intervention.

              Experimental, controlled, animal study. To evaluate the functional effect of hyperbaric oxygen therapy administered shortly, one day after, and no intervention (control) in standardized experimental spinal cord lesions in Wistar rats. São Paulo, Brazil. In all, 30 Wistar rats with spinal cord lesions were divided into three groups: one group was submitted to hyperbaric oxygen therapy beginning half an hour after the lesion and with a total of 10 one-hour sessions, one session per day, at 2 atm; the second received the same treatment, but beginning on the day after the lesion; and the third received no treatment (control). The Basso, Beattie and Bresnahan scales were used for functional evaluation on the second day after the lesion and then weekly, until being killed 1 month later. There were no significant differences between the groups in the functional analysis on the second day after the lesion. There was no functional difference comparing Groups 1 and 2 (treated shortly after or one day after) in any evaluation moment. On the 7th day, as well as on the 21st and 28th postoperative days, the evaluation showed that groups 1 and 2 performed significantly better than the control group (receiving no therapy). Hyperbaric chamber therapy is beneficial in the functional recovery of spinal cord lesions in rats, if it is first administered just after spinal cord injury or within 24 h.
