      Type-D Personality in Unemployed Subjects: Prevalence, Self-Efficacy and Heart Rate Variability/Autonomic Response

          Abstract

          Background:

          Unemployment may impair mental and physical health. The influencing factors causing such negative effects are relevant from an individual and public health perspective. The personality as one possible influencing factor was discussed. This study investigated the prevalence of the type-D personality in an unemployed population and its connections to socio-demographic, psychological and heart rate variability (HRV) parameters.

          Methods:

          A questionnaire set including socio-demographics, type-D scale (DS14), Complaint list (BL), Beck-Depression-Inventory II (BDI-II) and the General Self-Efficacy Scale (GSE) was handed out to 203 unemployed individuals [126 females, mean age ± SD: 42.36 ± 11.08]. For HRV assessment (RMSSD), a subsample of 83 participants [50 females, median age ± IQR: 47.00 ± 17.00] passed the “stress-tests” (timed breathing, d2-attention-stress-test, math test) while heart frequency (HF) was acquired viathe Stressball software (BioSign GmbH, Ottenhofen, Germany).

          Results:

          53% of the unemployed had a type-D personality. Compared to non-type-D individuals, type-D individuals had rarely children and by trend a lower educational level; they showed significantly higher scores in the BDI-II and lower scores in the GSE and BL. No differences were observed in mean HF or RMSSD during all the stress-tests.

          Conclusion:

          The HRV of individuals with a type-D personality is no worse than that of individuals without a type-D personality. Type-D personality was significantly associated with negative health effects regarding depressiveness, self-efficacy and physical complaints. Our main findings implicate that the DS14 could serve as a short and reliable screening instrument to select concerned unemployed individuals who might be at risk for negative health effects for adequate intervention.

            The ‘Trier Social Stress Test’ – A Tool for Investigating Psychobiological Stress Responses in a Laboratory Setting

            This paper describes a protocol for induction of moderate psychological stress in a laboratory setting and evaluates its effects on physiological responses. The 'Trier Social Stress Test' (TSST) mainly consists of an anticipation period (10 min) and a test period (10 min) in which the subjects have to deliver a free speech and perform mental arithmetic in front of an audience. In six independent studies this protocol has been found to induce considerable changes in the concentration of ACTH, cortisol (serum and saliva), GH, prolactin as well as significant increases in heart rate. As for salivary cortisol levels, the TSST reliably led to 2- to 4-fold elevations above baseline with similar peak cortisol concentrations. Studies are summarized in which TSST-induced cortisol increases elucidated some of the multiple variables contributing to the interindividual variation in adrenocortical stress responses. The results suggest that gender, genetics and nicotine consumption can influence the individual's stress responsiveness to psychological stress while personality traits showed no correlation with cortisol responses to TSST stimulation. From these data we conclude that the TSST can serve as a tool for psychobiological research.

              Impact of Reduced Heart Rate Variability on Risk for Cardiac Events: The Framingham Heart Study

              Although heart rate variability (HRV) is altered in a variety of pathological conditions, the association of reduced HRV with risk for new cardiac events has not been studied in a large community-based population. The first 2 hours of ambulatory ECG recordings obtained on subjects of the Framingham Heart Study who were free of clinically apparent coronary heart disease or congestive heart failure were reprocessed to assess HRV. Five frequency-domain measures and three time-domain measures were obtained. The associations between HRV measures and the incidence of new cardiac events (angina pectroris, myocardial infarction, coronary heart disease death, or congestive heart failure) were assessed with proportional hazards regression analyses. There were 2501 eligible subjects with a mean age of 53 years. During a mean follow-up of 3.5 years, cardiac events occurred in 58 subjects. After adjustment for age, sex, cigarette smoking, diabetes, left ventricular hypertrophy, and other relevant risk factors, all HRV measures except the ratio of low-frequency to high-frequency power were significantly associated with risk for a cardiac event (P = .0016 to .0496). A one-standard deviation decrement in the standard deviation of total normal RR intervals (natural log transformed) was associated with a hazard ratio of 1.47 for new cardiac events (95% confidence interval of 1.16 to 1.86). The estimation of HRV by ambulatory monitoring offers prognostic information beyond that provided by the evaluation of traditional cardiovascular disease risk factors.
