          Abstract

          Devido à associação freqüentemente apontada na literatura entre o fenômeno da migração e o desenvolvimento de distúrbios mentais, este trabalho tencionou fornecer o perfil psicológico de uma amostra de migrantes, por meio de uma forma reduzida de um instrumento de valor clínico e empírico comprovado: o MMPI. Para tanto, 20 migrantes do sexo masculino, analfabetos e sem comprometimentos intelectuais severos foram submetidos à forma IRF do MMPI, desenvolvida nos Estados Unidos em 1980, destinada a sujeitos com essas características cognitivas. Os resultados se constituíram em altas pontuações na escala F de validade, relacionadas principalmente a questões de ordem cultural e incompreensão dos itens. Em termos clínicos, dentre os protocolos considerados confiáveis para análise, foi detectada uma tendência da IRF a produzir elevações nos resultados das escalas clínicas de um modo generalizado, indicando problemas relativos à padronização, validade e capacidade discriminativa do instrumento para uso na população brasileira não alfabetizada.

          Translated abstract

          Due to the association, frequently pointed out by literature, between the phenomenon of human migration and the development of mental disturbances, this work intends to supply the psychological profile of a migrants` Brazilian sample, by means of a reduced form of an instrument which has had its clinical and empirical value proved: the MMPI. For this, 20 masculine migrants, illiterates and without severe intellectual handicaps were submitted to IRF form of MMPI, that was developped in the USA for individuals with those cognitive characteristics. The results showed high scores in the F scale, related mainly to cultural issues and misunderstanding of items. Among the protocols that seem to be reliable for analysis have been detected a general IRF tendency of producing high scores in clinical scales. This indicates difficulties related to IRF standartization, validity and discrimination used for the Brazilian illiterate population.

          A saúde dos idosos brasileiros às vésperas do século XXI: problemas, projeções e alternativas

          São analizadas as características atuais e as perspectivas dos processos de transição demográfica e epidemiológica no Brasil. Foram utilizados os resultados do censo demográfico de 1991 e 1996 e Pesquisa Nacional por Amostra de Domicílios (PNAD) de 1993 e 1995, de novas projeções populacionais, e de recentes estudos sobre o perfil socioeconômico e de saúde de idosos de três grandes capitais brasileiras. Embora se estime que a proporção de idosos deverá duplicar até 2050, alcançando 15% do total da população, doenças crônico-degenerativas e distúrbios mentais já têm determinado, atualmente, maciça utilização dos serviços de saúde. O desenvolvimento de doenças, incapacidades e dependência têm sido mais freqüentes dentre aqueles de baixa renda que, no entanto, não têm conseguido garantir a assistência social e de saúde que demandam. Ações preventivas devem ser coordenadas por unidades básicas de saúde, priorizando necessidades locais. É imprescindível o investimento imediato na saúde, educação e formação técnica dos jovens, nos programas de apoio aos familiares e na manutenção de idosos em atividades produtivas adequadas.

            Acculturative stress among young immigrants in Norway.

            The study examined the relationship between migration and the incidence of emotional disorders among 568 young Third World immigrants in Norway. Participants were 10-17 years of age. Using a questionnaire, acculturative stress (i.e., change in health status as a result of acculturation) was found to exist among the children, although having to migrate or being born in Norway was not related to mental health status. A stressful acculturative experience (i.e., difficulties in initiating friendship with Norwegian peers) alone could account for only 1% of the self reported emotional disorders. Incidence of depressive tendencies, poor self image, and psychological and somatic symptoms were found to be related to close and supportive parents, marginality, integration, gender and the number of friends the child had. These accounted for between 12 and 15% of the explained variance. The paper theoretically discusses how these factors may be related to acculturative stress, and recommends them as starting points for a primary intervention program to reduce emotional disorders among these children.

              Prevalence and magnitude of perinatal substance exposures in California.

              Perinatal substance exposure has been linked to many neonatal and obstetrical complications. There have been few population-based epidemiologic studies to identify the prevalence and demographic profiles associated with drugs, alcohol, and smoking during pregnancy. We studied a fully probabilistic stratified-cluster sample to estimate the prevalence of perinatal substance exposures in California in 1992. Urine samples from 29,494 women presenting for delivery in 202 hospitals were coded and screened for toxins; the results of toxicology screening were later linked by code number to the subjects' demographic variables and their reported use of tobacco and prescribed drugs. Urinary toxicologic tests provide conservative estimates because they can detect only very recent substance use. The weighted prevalence for perinatal substance exposure was 5.16 percent for the use of one or more drugs, 6.72 percent for alcohol (analyzed independently), and 8.82 percent for self-reported smoking. The percentage of women testing positive for any drug, including alcohol, was 11.35 percent. Estimates for racial and ethnic groups varied widely. Black women had the highest prevalence of total drug use (14.22 percent), alcohol use (11.58 percent), cocaine use (7.79 percent), and tobacco use (20.12 percent). Most drug exposures occurred among white non-Hispanic and Hispanic women. White non-Hispanic women had the second highest prevalence rate for the use of one or more drugs (6.79 percent) and self-reported tobacco use (14.82 percent). Hispanic women had the second highest prevalence rate for alcohol (6.87 percent). In California in 1992, there were 67,361 estimated perinatal exposures to one or more drugs, including alcohol, and 52,346 self-reported exposures to tobacco. These findings have clinical and public health implications.
