          Abstract

          ABSTRACT The ameloblastic fibro-odontoma is a benign, mixed, asymptomatic and rare odontogenic tumor that can easily be confused radiographically and histologically with other diseases. Usually it affects a population between the first and second decades of life, is more frequent in the mandible and shows predilection for males. This lesion, characterized by dental agenesis at the tumor site, has no predilection for anatomical region; however, an increase of intraoral volume may cause mild facial asymmetry. It shows slow but expansive growth, and is a well-defined radiolucent lesion, usually unilocular, with radiopacity inside. Normally surgical removal is conservative and the prognosis is favorable. This article reports a case of ameloblastic fibro-odontoma treated by surgical removal and follow-up of two years. The aim of this work was to study the ameloblastic fibro-odontoma in its entirety, seeking to inform clinicians about the best diagnostic and treatment methods for this type lesion.

          RESUMO O fibro-odontoma ameloblástico é um tumor odontogênico misto benigno, assintomático e raro, que pode ser facilmente confundido radiograficamente e histopatologicamente com outras enfermidades. Normalmente atinge uma população entre a primeira e a segunda décadas de vida, sendo mais frequente em mandíbula e com predileção pelo sexo masculino. Este tumor caracteriza-se pela agenesia dentária no local do tumor, sem predileção por região anatômica, e por um aumento de volume intrabucal, podendo causar leve assimetria facial. Apresenta crescimento lento, porém expansivo; revela-se como uma lesão radiolúcida bem definida, normalmente unilocular, com radiopacidades no seu interior. Normalmente a remoção cirúrgica é conservadora e o prognóstico é favorável. O presente trabalho relata um caso clínico de fibro-odontoma ameloblástico tratado por meio de remoção cirúrgica e com acompanhamento de dois anos. O objetivo deste trabalho foi estudar o fibro-odontoma ameloblástico em sua integralidade, buscando expor ao cirurgião-dentista os melhores métodos de diagnóstico e tratamento para tal lesão.

          Malignant transformation of ameloblastic fibro-odontoma to ameloblastic fibrosarcoma.

          Two cases of malignant transformation of ameloblastic fibro-odontomas are presented, along with a review of the literature on ameloblastic fibrosarcomas. The occurrence of this malignant transformation of ameloblastic fibromas, ameloblastic odontomas, and ameloblastic fibro-odontomas appears to be more frequent than previously thought. This potential transformation alone does not justify radical treatment of all these benign lesions. If there is recurrence accompanied by a histologic pattern change toward a more unorganized fibrous stroma with displacement of the epithelial component, however, then more extensive treatment procedures appear to be indicated.

            Clinical and radiological profile of ameloblastic fibro-odontoma: an update on an uncommon odontogenic tumor based on a critical analysis of 114 cases.

            Ameloblastic fibro-odontoma is an uncommon benign tumor of the jaws that belongs to the group of mixed odontogenic tumors. The descriptions of its clinical and radiological features in the literature are not always accurate and sometimes even contradictory. The aim of the present study was to critically evaluate their clinical and radiological features as reported in the English-language literature. A total of 114 well-documented cases of ameloblastic fibro-odontomas (103 from publications and 11 of our own new cases) were analyzed. The patients' age ranged from 8 months to 26 years (mean 9.6). There were 74 (65 %) males, with a male-to-female ratio of 1.85:1 (P = 0.001). The mandible was involved in 74 (65 %) cases, and the mandible-to-maxilla ratio was 1.85:1 (P < 0.001). Nearly 80 % of the lesions were located in the posterior region of the jaws, and most (58 %) were in the posterior mandible. Radiographically, most of the lesions were unilocular and only a few (~10 %) were multilocular. Most lesions were mixed radiolucent-radiopaque, and only a few (~5 %) were radiolucent. Almost all lesions (~92 %) were associated with the crown of an unerupted tooth/teeth. This comprehensive analysis of a large number of patients with an uncommon lesion revealed that ameloblastic fibro-odontomas are significantly more common in males and in the mandible, and that multilocular lesions are uncommon. It also revealed that, based on their clinical and radiological features, some of them are probably true neoplasms while others appear to be developing odontomas (hamartomas).

              Ameloblastic fibro-odontoma: a case report.

              Ameloblastic fibro-odontoma (AFO) is a rare, benign, slow-growing odontogenic tumor, generally asymptomatic and more prevalent in children and adolescents. We report a case of AFO in the mandible of an eight-year-old Caucasian male patient, and review the literature . Intraoral examination revealed a swelling extending from the deciduous second molar to the retromolar triangle, covered with normal mucosa. A panoramic radiograph showed a large, well-demarcated radiolucency with radiopaque areas. The provisional diagnosis was of AFO, and so an incisional biopsy was performed. Histologically, the lesion was composed of connective tissue resembling the dental papilla, with epithelial strands or islands, as well as denticles and amorphous masses of enamel and dentin consistent with a diagnosis of AFO. Surgical excision and curettage of the lesion were performed. The patient has been monitored for eight years and the lesion has not recurred.
