          Abstract

          Resumo Introdução O sangue, seus componentes e derivados são imprescindíveis para o funcionamento da atenção hematológica e transfusional nos sistemas de saúde modernos. Objetivo Identificar algumas das dificuldades, dos desafios e das estratégias que os serviços de hemoterapia públicos experimentam no momento atual, a partir de perspectivas regionais na Espanha e no Brasil. Método Foram realizadas entrevistas com onze gestores/profissionais de serviços de hemoterapia das Comunidades Autônomas de Madri, Extremadura e Catalunha (Espanha) e do Estado de Bahia (Brasil). Resultados Identificaram-se dois grupos de dificuldades: disposição de estrutura e recursos; e recrutamento e fidelização dos doadores. Desafios: envolver e fidelizar novos doadores, sobretudo jovens; manter estoque suficiente de sangue de tipos raros e dispor de hemoderivados; lidar com patologias emergentes decorrentes do processo de migração e globalização; assegurar o uso racional do sangue; gerir os serviços com restrição sobre os recursos financeiros; lidar com distintos atores. Como linhas estratégicas, destacaram-se: uso de diversos recursos para sensibilização e fidelização de doadores; concentração tecnológica e de procedimentos nos centros coordenadores; implantação de programas públicos de plasmaférese; utilização de diferentes meios para estabelecer as relações com distintos atores. Conclusão Salvaguardadas as peculiaridades regionais, os resultados podem ser utilizados para a reflexão e implementação de variadas ações voltadas para a doação e autossuficiência em outras regiões do mundo.

          Translated abstract

          Abstract Background Blood, its components and derivatives are essential for the functioning of hematological and transfusion care in modern health systems. Objective To identify some of the difficulties, challenges, and strategies experienced in public blood banks currently, based on regional perspectives in Spain and Brazil. Method Interviews with eleven managers/professionals from blood banks of the Autonomous Communities of Madrid, Extremadura and Catalonia (Spain) and the State of Bahia (Brazil) were conducted. Results Two groups of difficulties were identified: availability of structure and resources; and recruitment and loyalty of donors. Challenges: to recruit and retain new donors, especially young people; to maintain sufficient stock of rare blood types and have blood products available; to deal with emerging pathologies arising from migration and globalization; to ensure the rational use of blood; to manage services with limited autonomy over financial resources; to deal with different stakeholders. The following strategic lines stood out: the use of several resources to raise awareness and loyalty of donors; the concentration of technology and procedures in coordinating centers; the implantation of public plasmapheresis programs and the utilization of different means to establish relations with different actors. Conclusion Respecting the regional peculiarities, the results can be used for the reflection and implementation of various actions aimed at blood donation and self-sufficiency in other regions of the world.

              Blood donation as a public good: an empirical investigation of the free rider problem.

              A voluntary blood donation system can be seen as a public good. People can take advantage without contributing and have a free ride. We empirically analyse the extent of free riding and its determinants. Interviews of the general public in Spain (n = 1,211) were used to ask whether respondents were (or have been) regular blood donors and, if not, the reason. Free riders are defined as those who are medically capable to donate blood but do not. In addition, we distinguish four different types of free riding depending on the reason given for not donating. Binomial and multinomial logit models estimate the effect of individual characteristics on the propensity to free ride and the likelihood of the free rider types. Amongst those who are able to donate, there is a 67 % probability of being a free rider. The most likely free rider is female, single, with low/no education and abstained from voting in a recent national election. Gender, age, religious practice, political participation and regional income explain the type of free rider.
