          Abstract

          No Brasil e no mundo a incidência do câncer de mama vem aumentando e aparecendo cada vez mais cedo na vida da mulher. O tratamento envolve mastectomia, quimioterapia e radioterapia, que, pelos seus efeitos físicos, podem comprometer em variados graus a auto-estima, a imagem corporal e a identidade feminina daquelas que recebem o diagnóstico da doença. Além disso, em nossa sociedade o câncer adquiriu significados relacionados a culpa, punição, deterioração, dor e morte, agravando o sofrimento psicológico das doentes. Neste sentido, este artigo tem o objetivo de levantar alguns pontos de reflexão no que se relaciona aos significados culturais da doença e do significado do seio enquanto ícone da identidade feminina. Partimos do princípio de que enriquecer a compreensão dos aspectos que influenciam o sofrimento da mulher com câncer de mama contribui para que os profissionais de saúde, principalmente os de saúde mental, possam assisti-la de maneira mais eficiente e abrangente.

          En Brasil y en el mundo la incidencia del cáncer de mama está en acenso. Es una enfermedad que está manifestándose cada vez más temprano en la vida de la mujer. El tratamiento envuelve mastectomía, quimioterapia y radioterapia que, por sus efectos físicos, pueden comprometer en variados grados la autoestima, la imagen corporal y la identidad femenina de aquellas mujeres que reciben el diagnóstico de la enfermedad. Además de eso, en nuestra sociedad el cáncer ha adquirido significados relacionados con la culpa, punición, deterioración, dolor y muerte, agravando el sufrimiento psicológico de las enfermas. En este sentido, este artículo tiene como objetivo levantar algunos puntos de reflexión en lo que se relaciona con los significados culturales de la enfermedad y del significado del seno como icono de la identidad femenina. Partimos del supuesto de que enriquecer la comprensión de los aspectos que pueden influenciar el sufrimiento de la mujer con cáncer de mama, contribuye para que los profesionales de la salud, sobretodo los de salud mental, puedan prestarle asistencia de un modo más eficiente y abarcador.

          The incidence of breast cancer in Brazil and all over the world has been rising and affected women at an earlier age. Treatment involves mastectomy, chemotherapy and radiotherapy whose side effects might compromise self-esteem, body image and feminine identity of the sick to various degrees. Moreover, in current society, cancer has acquired meanings associated with guilt, punishment, deterioration, pain and death, aggravating the patients’ psychological suffering. Therefore, current essay reflects on the cultural meanings of the disease and on the breast as an icon of feminine identity. The basic assumption is that knowledge on the aspects influencing the suffering of breast-cancer-sick women is a great help for health professionals, particularly those who care for mental health, to assist their patients in a more effective and comprehensive way.

          Quality of life at the end of primary treatment of breast cancer: first results from the moving beyond cancer randomized trial.

          During the last decade, survival rates for breast cancer have increased as a result of earlier detection and increased use of adjuvant therapy. Limited data exist on the psychosocial aspects of the transitional period between the end of primary treatment and survivorship. We investigated the baseline psychosocial status of women enrolled in a randomized trial testing two psychosocial interventions for women at the end of primary treatment. Participants, identified within 1 month after surgery (registration), provided demographic information and limited measures of quality of life. They were followed until they finished primary treatment (enrollment), at which time they completed a mailed baseline survey that included standardized measures of quality of life (including standardized scales of physical and emotional functioning), mood, symptoms, and sexual functioning. A total of 558 patients (mean age = 56.9 years) were enrolled in the study between July 1, 1999, and June 30, 2002. Health outcomes were examined according to treatment received: mastectomy with and without chemotherapy, and lumpectomy with and without chemotherapy. All statistical tests were two-sided. Among all treatment groups, patients who had a mastectomy had the poorest physical functioning at registration (P<.001) and at enrollment (P=.05). At enrollment, mood and emotional functioning were similar among all patients, with no differences by type of treatment received. At enrollment, symptoms, including muscle stiffness, breast sensitivity, aches and pains, tendency to take naps, and difficulty concentrating, were common among patients in all groups and were statistically significantly associated with poor physical functioning and emotional well-being. Sexual functioning was worse for women who received chemotherapy than for those who did not, regardless of type of surgery (P<.001). At the end of primary treatment for breast cancer, women in all treatment groups report good emotional functioning but report decreased physical functioning, particularly among women who have a mastectomy or receive chemotherapy. Clinical interventions to address common symptoms associated with treatment should be considered to improve physical and emotional functioning at the end of primary treatment for breast cancer.

              How coping mediates the effect of optimism on distress: a study of women with early stage breast cancer.

              At diagnosis, 59 breast cancer patients reported on their overall optimism about life; 1 day presurgery, 10 days postsurgery, and at 3-, 6-, and 12-month follow-ups, they reported their recent coping responses and distress levels. Optimism related inversely to distress at each point, even controlling for prior distress. Acceptance, positive reframing, and use of religion were the most common coping reactions; denial and behavioral disengagement were the least common reactions. Acceptance and the use of humor prospectively predicted lower distress; denial and disengagement predicted more distress. Path analyses suggested that several coping reactions played mediating roles in the effect of optimism on distress. Discussion centers on the role of various coping reactions in the process of adjustment, the mechanisms by which dispositional optimism versus pessimism appears to operate, third variable issues, and applied implications.
