          Abstract

          ABSTRACT Objective: To analyze the patient safety climate in intensive care units from the nursing perspective. Methods: Cross-sectional study developed with 87 nursing professionals working in three Intensive Care Units of a public hospital for emergency services in Piauí from October to November 2018. The study used a validated Safety Attitudes Questionnaire (SAQ). In the inferential analysis, the Student’s t-test, Mann-Whitney, and Kruskal-Wallis were performed. Results: The total SAQ score obtained a mean of 68.57. In analyzing the scores obtained for each domain in the general SAQ, the Job Satisfaction, and Teamwork Climate domains were those that obtained the highest scores, and the lowest score was for the Perception of Hospital Management domain. Conclusion: The safety attitudes assessed from the perspective of the nursing team proved to be unfavorable.

          RESUMEN Objetivo: Analizar el clima de seguridad del paciente en unidades de terapia intensiva en la perspectiva de la enfermaría. Métodos: Estudio transversal desarrollado con 87 profesionales de enfermaría que actúan en tres Unidades de Terapia Intensiva de un hospital público de urgencia de Piauí en el período de octubre y noviembre de 2018. Ha sido utilizado un cuestionario validado de Actitudes de Seguridad – Safety Attitudes Questionnaire (SAQ). En el análisis inferencial, han sido realizados el Teste t de Student, Mann-Whitney e Kruskal-Wallis. Resultados: La calificación del SAQ total obtuvo media de 68,57. En análisis de las calificaciones obtenidas para cada dominio en el SAQ general, los dominios Satisfacción en el Trabajo y Clima de Trabajo en Equipo han sido los que obtuvieron las calificaciones más altas y la calificación más baja ha sido para el dominio Percepción de la Gerencia del Hospital. Conclusión: Las actitudes de seguridad evaluadas bajo la perspectiva del equipo de enfermaría se ha mostrado desfavorables.

Utilizou-se um questionário validado de Atitudes de Segurança – Safety Attitudes Questionnaire (SAQ). Na análise inferencial, foram realizados o Teste t de Student, Mann-Whitney e Kruskal-Wallis. Resultados: O escore do SAQ total obteve média de 68,57. Em análise dos escores obtidos para cada domínio no SAQ geral, os domínios Satisfação no Trabalho e Clima de Trabalho em Equipe foram os que obtiveram os escores mais altos e o escore mais baixo foi para o domínio Percepção da Gerência do Hospital. Conclusão: As atitudes de segurança avaliadas sob a perspectiva da equipe de enfermagem mostraram-se desfavoráveis.

          Cultura e clima organizacional para segurança do paciente em Unidades de Terapia Intensiva

          RESUMO Objetivo Avaliar a percepção dos profissionais de saúde sobre o clima e a cultura de segurança do paciente em Unidades de Terapia Intensiva (UTI) e a relação entre os instrumentos Hospital Survey on Patient Safety Culture (HSOPSC) e o Safety Attitudes Questionnaire (SAQ). Método Estudo transversal realizado em hospital de ensino no interior do estado de São Paulo, Brasil, em março/abril de 2014. Aplicaram-se o HSOPSC, o SAQ e um instrumento para levantamento das informações sociodemográficas e profissionais aos funcionários das UTI adulto, pediátrica e neonatal. A análise utilizou a estatística descritiva. Resultados As escalas apresentaram boa confiabilidade. Maiores fragilidades para a segurança do paciente foram observadas nos domínios “condições de trabalho” e “percepções da gerência” do SAQ e “resposta não punitiva aos erros” do HSOPSC. As fortalezas no SAQ foram o “clima de trabalho em equipe” e a “satisfação no trabalho” e para o HSOPSC “expectativas e ações de promoção de segurança supervisores/gerentes” e “aprendizado organizacional e melhoria mútua”. Na UTI Neonatal houve maior satisfação no trabalho do que nas demais UTI. A UTI Adulto apresentou menores pontuações para a maioria dos domínios do SAQ e HSOPSC. A correlação entre as escalas foi de força moderada (r=0,66). Conclusão Há diferenças de percepções quanto à segurança do paciente entre as UTI, o que corrobora com a existência de microculturas locais. O estudo não demonstra que o SAQ e o HSOPSC sejam equivalentes.

            Interprofessional team assessments of the patient safety climate in Swedish operating rooms: a cross-sectional survey

            Background A positive patient safety climate within teams has been associated with higher safety performance. The aim of this study was to describe and compare attitudes to patient safety among the various professionals in surgical teams in Swedish operating room (OR) departments. A further aim was to study nurse managers in the OR and medical directors’ estimations of their staffs’ attitudes to patient safety. Methods A cross-sectional survey with the Safety Attitudes Questionnaire (SAQ) was used to elicit estimations from surgical teams. To evoke estimations from nurse managers and medical directors about staff attitudes to patient safety, a short questionnaire, based on SAQ, was used. Three OR departments at three different hospitals in Sweden participated. All licensed practical nurses (n=124), perioperative nurses (n=233), physicians (n=184) and their respective manager (n=22) were invited to participate. Results Mean percentage positive scores for the six SAQ factors and the three professional groups varied, and most factors (safety climate, teamwork climate, stress recognition, working conditions and perceptions of management), except job satisfaction, were below 60%. Significantly lower mean values were found for perioperative nurses compared with physicians for perceptions of management (56.4 vs 61.4, p=0.013) and working conditions (63.7 vs 69.8, p=0.007). Nurse managers and medical directors’ estimations of their staffs’ ratings of the safety climate cohered fairly well. Conclusions This study shows variations and some weak areas for patient safety climate in the studied ORs as reported by front-line staff and acknowledged by nurse managers and medical directors. This finding is a concern because a weak patient safety climate has been associated with poor patient outcomes. To raise awareness, managers need to support patient safety work in the OR.

              Integrating Patient Safety Education in the Undergraduate Nursing Curriculum: A Discussion Paper

              Background: This paper explores the opportunities and challenges for integrating patient safety education in undergraduate nursing curriculum. Methods: Four dimensions of undergraduate nursing education are examined: National accreditation of nursing programs, building a competency-based nursing education, a model of nursing education and building faculty capacity in patient safety education and research. Results: Incorporating patient safety in a nursing curriculum can be “institutionalized” by making it a pre-requisite for granting program accreditation. At the operational level, transforming undergraduate nursing education to incorporate inquiry-based learning and moving toward competency-based patient safety education are two key requirements for engaging the students with patient safety science. Building faculty capacity who are experts in both patient safety teaching and research remains a key challenge that needs to be addressed to enable a shift in the patient safety “mindset” for future nursing workforce. Conclusion: Efforts to introduce patient safety in nursing education are both necessary and timely, and should accommodate students’ unique needs and cultural context.
