          Abstract

          As repercussões emocionais do câncer podem atingir o cônjuge com quase a mesma intensidade que atingem o paciente, justificando um paralelo com a noção de trauma para a Psicanálise. Já a capacidade dos cônjuges em ressignificar este trauma permite uma aproximação com o conceito de resiliência. O presente estudo transversal, qualitativo, exploratório e descritivo objetivou compreender a perspectiva do cônjuge sobre o adoecimento por câncer do(a) parceiro(a), analisando sua relação com os conceitos de trauma, ressignificação e resiliência para a Psicanálise. Entrevistas semiestruturadas foram realizadas com 11 cônjuges e analisadas por meio da Análise de Conteúdo Temática, obtendo-se 4 eixos temáticos (Complexidade emocional da vivência; Acesso ao traumático; Vicissitudes do processo de ressignificação; Expectativas para o futuro). Concluiu-se que, devido à complexidade da vivência, o reconhecimento do trauma e sua ressignificação nem sempre são possíveis, indicando a importância dos cônjuges como objetos de cuidado e estudo dos profissionais da saúde.

          Translated abstract

          The emotional repercussions of cancer can reach the spouse with almost the same intensity as the patient, justifying a parallel with the notion of trauma for Psychoanalysis. The spouses' ability to reframe this trauma allows an approximation with the concept of resilience. This cross-sectional, qualitative, exploratory and descriptive study aimed to understand the perspective of the spouse about the partner's cancer illness, analyzing its relation with the concepts of trauma, resignification and resilience for Psychoanalysis. Semi-structured interviews were conducted with 11 spouses and analyzed through Thematic Content Analysis, obtaining 4 thematic axes (Emotional complexity of experience; Access to traumatic; Vicissitudes of the resignification process; Expectations for the future). It was concluded that, due to the complexity of the experience, the recognition of trauma and its resignification are not always possible, indicating the importance of spouses as objects of care and study of health professionals.

          The concept of family resilience: crisis and challenge.

          The concept of resilience, the ability to withstand and rebound from crisis and adversity, has valuable potential for research, intervention, and prevention approaches aiming to strengthen couples and families. Resilience has been viewed as residing within the individual, with the family often dismissed as dysfunctional This article advances a systemic view of resilience in ecological and developmental contexts and presents the concept of family resilience, attending to interactional processes over time that strengthen both individual and family hardiness. Extending our understanding of normal family functioning, the concept of family resilience offers a useful framework to identify and fortify key processes that enable families to surmount crises and persistent stresses. There are many pathways in relational resilience, varying to fit diverse family forms, psychosocial challenges, resources, and constraints. Shared beliefs and narratives that foster a sense of coherence, collaboration, competence, and confidence are vital in coping and mastery. Interventions to strengthen family resilience have timely relevance for weathering the rapid social changes and uncertainties facing families today.

            Parents of Children With Cancer: At-Risk or Resilient?

            Objective  To examine adjustment in parents of children with cancer using a design that minimizes focusing effects and allows for direct comparison with parents of healthy children. Method  Parents of 305 children with cancer and a demographically similar sample of 231 parents of healthy children were evaluated using diagnostic interviews for posttraumatic stress disorder (PTSD), and questionnaire measures of posttraumatic stress symptoms (PTSS) and psychological growth (PG), as well as measures of global psychological functioning.  Results  Rates of current and lifetime PTSD in parents of children with cancer were low, and did not differ from comparison parents. Likewise, levels of PTSS were not significantly different from comparison parents, but differed as a function of time since diagnosis, with parents of children who were ≥5 years from diagnosis reporting significantly lower PTSS than comparison parents. PG was higher in parents of children with cancer than in comparison parents regardless of time since diagnosis. Conclusion  Parents of children with cancer demonstrate resilience to this challenge.

              Sobrecarga no cuidar e suas repercussões nos cuidadores de pacientes em fim de vida: revisão sistemática da literatura

              Resumo Cuidar de um familiar com doença avançada e/ou em fim de vida pode representar uma grande sobrecarga emocional, física e financeira que afeta a qualidade de vida dos cuidadores. O presente estudo teve como objetivo realizar uma revisão sistemática da literatura sobre a sobrecarga no cuidar, os fatores relacionados e suas consequências nos cuidadores de pacientes com câncer avançado em fim de vida ou em cuidados paliativos. Foi realizada uma busca de artigos científicos publicados nas bases de dados EBSCO, Web of Knowledge e Bireme, desde os primeiros registros nas respectivas bases de dados sobre o tema até março de 2014. Dos 582 artigos encontrados, apenas 27 foram selecionados. A maioria dos artigos afirma que os cuidadores familiares estão sobrecarregados. Em alguns estudos, a sobrecarga no cuidar aparece associada a características do paciente e da sua doença; em outros, a um pior estado de saúde do cuidador, a uma maior sintomatologia psicopatológica (ansiedade, depressão, distress emocional) e também ao desenvolvimento de complicações no luto. Porém, a esperança, o apoio social, a capacidade do cuidador de atribuir um significado à experiência de cuidar e se sentir confortável com as tarefas de cuidar foram associados a menores níveis de sobrecarga.
