      Surgical Management of Periapical Lesion with Dens in Dente

          Abstract

          The management of one case of dens in Dente (Dens invaginatus) in maxillary lateral incisor with history of trauma to maxillary central incisor with periradicular lesion is reported. The patient presented with pain and fracture of anterior tooth. Despite of complex anatomy and diagnosis of dens invaginatus, surgical root canal (Apicoectomy) was performed successfully. Further more essential clinical considerations and treatment options are suggested. Early diagnosis and management are important to avoid complications.

          A 19-yr-old female presented with three root canals and dens formation in a maxillary right lateral incisor. This rare condition was treated with conventional endodontic therapy.

            Endodontic treatment of dens invaginatus: a 5-year follow-up.

            Dens invaginatus is an anomaly of the tooth formation of embryonic origin that presents itself in several morphologic types. The complex anatomy of these teeth makes nonsurgical endodontic treatment complex and more so when its apex is immature. The 2 cases reported illustrate the nonsurgical endodontic management of a dens invaginatus type II and type III with an immature apex and periapical lesions, in which mineral trioxide aggregate (MTA) in one case, and calcium hydroxide in the other one, were the materials used. A 5-year follow-up of both cases shows a complete periapical healing with bone formation at the site of the lesions.

              Root canal treatment of an invaginated maxillary lateral incisor with a C-shaped canal.

              The endodontic treatment of an atypical maxillary lateral incisor that contained a C-shaped root canal is described. The unusual anatomic configuration of this particular tooth and the advantage of using image magnification to facilitate endodontic treatment are discussed.
